益气养血通脉法治疗阵发性房颤的临床观察

摘　　要：目的探讨益气养血通脉法治疗阵发性房颤患者的疗效。方法60例阵发性房颤患者,采用随机数字表法分为对照组和治疗组,各30例。两组均给予常规抗心律失常西药(琥珀酸美托洛尔片)治疗,治疗组在此基础上应用中医益气养血通脉法治疗,3个月(12周)后比较两组患者中医证候、动态心电图改善效果,治疗前后P波离散度(Pd)及左房内径(LAd)。结果治疗组中医证候总有效率为93.33%,高于对照组的73.33%,差异有统计学意义(P<0.05)。治疗组动态心电图总有效率为96.67%,高于对照组的76.67%,差异有统计学意义(P<0.05)。治疗前,两组Pd及LAd比较,差异无统计学意义(P>0.05)。治疗后,两组Pd、LAd均较治疗前下降,且治疗组Pd(34.41±3.74)ms、LAd(37.00±3.25)mm低于对照组的(38.39±4.32)ms、(41.00±3.76)mm,差异有统计学意义(P<0.05)。结论益气养血通脉法治疗阵发性房颤疗效确切,在改善阵发性房颤患者的症状及预防复发方面疗效明显,值得临床推广应用。Objective To explore the efficacy of supplementing qi,nourishing blood and promotion of blood circulation therapy for paroxysmal atrial fibrillation.Methods 60 patients with paroxysmal atrial fibrillation were divided into a control group and a treatment group according to random numerical table,with 30 cases in each group.Both groups were treated with conventional anti-arrhythmic western medicine(metoprolol succinate tablets).On this basis,the treatment group was treated with traditional Chinese medicine(TCM)method of supplementing qi,nourishing blood and promotion of blood circulation.After 3 months(12 weeks),the improvement effect of traditional Chinese medicine syndrome and dynamic electrocardiogram,P-wave dispersion(Pd)and left atrial diameter(LAd)before and after treatment of the two groups were compared.Results The total effective rate of traditional Chinese medicine syndrome in the treatment group was 93.33%,which was higher than 73.33%in the control group,and the difference was statistically significant(P<0.05).The total effective rate of Holter electrocardiogram in the treatment group was 96.67%,which was higher than 76.67%in the control group,and the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in Pd and LAd between the two groups(P>0.05).After treatment,Pd and LAd decreased in both groups compared with those before treatment;Pd and LAd in the treatment group were(34.41±3.74)ms and(37.00±3.25)mm,which were lower than(38.39±4.32)ms and(41.00±3.76)mm in the control group;the difference was statistically significant(P<0.05).Conclusion Supplementing qi,nourishing blood and promotion of blood circulation therapy is effective in the treatment of paroxysmal atrial fibrillation,and has obvious curative effect in improving the symptoms of patients with paroxysmal atrial fibrillation and preventing recurrence,which is worthy of clinical application.
