地特胰岛素联合阿卡波糖治疗老年T2DM患者早期糖尿病肾病的效果

摘　　要：目的研究对于老年T2DM患者早期糖尿病肾病应用地特胰岛素与阿卡波糖联合治疗的价值与效果。方法依据随机数字表法的形式将2022年2月至2023年6月诊治的94例老年T2DM早期糖尿病肾病患者进行分组,每组47例,对照组应用地特胰岛素治疗,试验组应用地特胰岛素联合阿卡波糖治疗,统计两组患者的临床治疗效果、血糖水平、肾功能指标、氧化应激指标。结果试验组糖化血红蛋白(HbA1c)、空腹血糖(FPG)、餐后2 h血糖(2 hPG)水平均低于对照组;试验组内生肌酐清除率(Ccr)高于对照组,包括血肌酐(SCr)、血尿素氮(BUN)、24 h尿蛋白均低于对照组;试验组超氧化物歧化酶(SOD)高于对照组,丙二醛(MDA)及晚期氧化蛋白产物(AOPPS)低于对照组;试验组临床治疗总有效率优于对照组,差异有统计学意义(P<0.05)。结论老年T2DM早期糖尿病肾病患者在地特胰岛素与阿卡波糖联合作用下能有效降低其血糖水平,改善肾功能指标,减轻氧化应激反应,可提升临床疗效。Objective To study the value and effect of combination therapy of insulin detemir and acarbose in elderly T2DM patients with early diabetes nephropathy.Methods In this paper,94 elderly T2DM patients with early diabetes nephropathy diagnosed and treated from February 2022 to June 2023 were grouped in the form of random number table.Each group included 47 patients.The control group was treated with insulin detemir,and the experimental group was treated with insulin detemir combined with acarbose.The clinical treatment effect,blood glucose level,renal function indicators,and oxidative stress indicators of the two groups were statistically verified.Results The levels of glycosylated hemoglobin(HbA1c),fasting blood glucose(FPG)and 2 h postprandial blood glucose(2 hPG)in experimental group were lower than those in control group;The endogenous creatinine clearance(Ccr)in experimental group was higher than that of the control group,and the serum creatinine(SCr),blood urea nitrogen(BUN)and 24 h urinary protein were lower than that of the control group;Superoxide dismutase(SOD)in experimental group was higher than control group,malondialdehyde(MDA)and advanced protein oxidation products(AOPPS)were lower than control group;The total effective rate of clinical treatment in experimental group was superior to the control group,the difference was statistically significant(P<0.05).Conclusions The combination of insulin dete and acarbose in elderly T2DM patients with early diabetes nephropathy can effectively reduce their blood sugar level,improve renal function indicators,reduce oxidative stress reaction,and improve clinical efficacy.
