心理社会因素对孕产妇产后并发症及胎儿心理生理影响研究

摘　　要：目的主要探讨心理社会因素对于孕产妇产后并发症及胎儿心理生理的影响。方法选择2020年11月至2022年12月收治的105例孕产妇。根据孕产妇胎心率分成正常组(n=74)与异常组(n=31);根据产后有无并发症分成有并发症组(n=22)、无并发症组(n=83)。协助孕产妇在孕早期、孕中期、孕晚期完成一般情况问卷、PHQ-9抑郁症筛查量表、生活事件量表(LES)、艾森克个性问卷(EPQ)、症状自评量表(SCL-90)问卷调查,在产后42 d内完成状态焦虑量表(SAI)、PHQ-9抑郁症筛查量表、爱丁堡产后抑郁量表(EPDS)问卷调查。结果胎心率异常组生活事件各因子得分及总值均高于正常组(P<0.05);孕产妇产前血压、胎心率与心理社会因素存在一定的关系;有并发症组的孕产妇,PHQ-9总分、SAI总分、EPDS总分均高于无并发症组(P<0.05)。结论针对孕产妇群体,心理社会因素影响孕产妇血压及胎儿预后,需加强关注。心理社会因素影响孕产妇及胎儿的健康。Objective To explore the influence of psychosocial factors on maternal and fetal psychophysiology.Methods One hundred and five pregnant women treated from November 2020 to December 2022 were selected.The pregnant women were divided into normal group(n=74)and abnormal group(n=31).According to postpartum complications,the patients were divided into two groups:complication group(n=22)and uncomplication group(n=83).General situation questionnaire,PHQ-9 Depression Screening Scale,Life Event Scale(LES),Eysenck Personality Questionnaire(EPQ)and Symptom Self-Rating Scale(SCL-90)questionnaires were completed for pregnant women in the first,second and third trimesters.State Anxiety Scale(SAI),PHQ-9 Depression Screening Scale and Edinburgh Postnatal Depression Scale(EPDS)were completed within 42 days after delivery.Results The scores and total values of life events in abnormal fetal heart rate group were higher than those in normal group(P<0.05).Prenatal blood pressure and fetal heart rate are related to psychosocial factors.The total scores of PHQ-9,SAI and EPDS in the complication group were higher than those in the non-complication group(P<0.05).Conclusions For pregnant women,psychosocial factors affect maternal blood pressure and fetal prognosis,which should be paid more attention.Psychological factors affect the health of pregnant women and fetuses.
