重症肺炎呼吸机相关性肺炎患者的改良集束化模式护理干预作用

摘　　要：目的研究机械通气治疗期间合并呼吸机相关性肺炎的重症肺炎患者,接受改良集束化护理干预的临床价值。方法选择2021年1月至2023年6月,在我院接受机械通气治疗期间,出现呼吸机相关性肺炎的重症肺炎患者,共计80例,采取随机分组法,将其分成对照组、观察组。对照组实施常规机械通气护理;观察组实施改良集束化护理。比较两组护理前后血气分析、炎性反应指标、心理状态评分、机械通气时间、呼吸功能恢复时间、住院总时间、不良反应发生率、护理满意度。结果观察组护理后血气分析、炎性反应指标、心理状态评分改善幅度优于对照组,组间数据比较有统计学意义(P<0.05);观察组机械通气时间、呼吸功能恢复时间、住院总时间短于对照组,组间数据比较有统计学意义(P<0.05);观察组不良反应发生率低于对照组,组间数据有统计学意义(P<0.05);观察组护理满意度高于对照组,组间数据比较有统计学意义(P<0.05)。结论机械通气治疗期间合并呼吸机相关性肺炎的重症肺炎患者,接受改良集束化护理干预,可改善血气分析指标,控制炎性反应,减少不良反应,缩短机械通气和住院总时间,以便保持良好心态,使护理满意度提高。Objective To study the clinical value of improved cluster nursing intervention in patients with severe pneumonia complicated with ventilator-associated pneumonia during mechanical ventilation.Methods 80 patients with severe pneumonia who developed ventilator-associated pneumonia during the period of from January 2021 to June 2023 mechanical ventilation were randomly divided into two groups,they were divided into control group and observation group.The control group was given routine mechanical ventilation nursing,and the observation group was given improved cluster nursing.The blood gas analysis,inflammatory reaction index,mental state score,mechanical ventilation time,respiratory function recovery time,total hospitalization time,adverse reaction rate and nursing satisfaction were compared between the two groups before and after nursing.Results After care,the improvement of blood gas analysis,inflammatory reaction index and psychological status score in the observation group was superior to than that in the control group(P<0.05).The time of mechanical ventilation,the time of recovery of respiratory function and the total time of hospitalization in the observation group were shorter than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05),the satisfaction degree of the observation group was higher than that of the control group(P<0.05).Conclusions the patients with severe pneumonia complicated with ventilator-associated pneumonia during mechanical ventilation treatment can receive improved cluster nursing intervention,which can improve blood gas analysis indexes,control inflammatory reaction and reduce adverse reactions,shorten the duration of mechanical ventilation and hospitalization in order to maintain a good state of mind,so as to improve nursing satisfaction.
