“肾主骨”理论辨治强直性脊柱炎继发骨质疏松症

摘　　要：强直性脊柱炎是常见的风湿病之一,主要影响中轴关节,中晚期可能会发生中轴关节的骨化,导致脊柱关节的强直与畸形。骨质疏松症是强直性脊柱炎常见的继发症之一,会加重患者关节畸形或导致脊柱椎体骨折,但在实际诊疗过程中强直性脊柱炎继发骨质疏松症并未引起足够的重视,缺乏明确的治疗方法与临床指南。传统医学理论认为肾主骨,肾精充足则骨骼强健有力,肾精不足则导致骨髓空虚,督脉不通发生强直性脊柱炎,骨髓失养,髓减骨枯进一步则继发骨质疏松症。故本文以“肾主骨”理论为指导对本病分期论治,在该理论视角下阐述中医药对强直性脊柱炎继发骨质疏松症的干预作用,体现其临床指导和应用价值。Ankylosing spondylitis is one of the common rheumatic diseases.It is an arthritis that mainly affects the axial joints.The axial joints may become ossified in the middle and late stage,leading to the stiffness and deformity of the spinal joints.Osteoporosis is one of the common secondary diseases,which aggravates joint deformation or leads to vertebral fractures.However,in the actual treatment process,the secondary osteoporosis of ankylosing spondylitis has not attracted enough attention,and there is a lack of a clear treatment method and clinical guideline.According to TCM theory,it is believed that the kidney dominates the bone.If the kidney essence is sufficient,the bone is strong,If the kidney essence is deficient,the bone marrow is empty.The obstruction of the artery leads to ankylosing spondylitis,bone marrow loss and bone loss,and secondary osteoporosis.Therefore,in this paper,the stage treatment of this disease is guided by the kidney dominating bone theory,and the intervention effect of traditional Chinese medicine on osteoporosis secondary to ankylosing spondylitis is expounded from the perspective of this theory,reflecting its clinical application value.
