直肠癌术后肛周感染血常规变化及VEGF和sTREM-1与PGE2的预测价值

摘　　要：目的探究直肠癌术后患者肛周感染血常规变化情况及血管内皮生长因子(VEGF)、可溶性髓系细胞触发受体1(sTREM-1)、前列腺素E2(PGE2)的预测价值。方法回顾性选取2019年2月-2023年2月河北省中医院60例直肠癌术后发生肛周感染患者的临床资料作为研究组,选取82例同期直肠癌术后未发生肛周感染患者的临床资料作为对照组。统计并分析两组患者的临床资料,比较两组患者血常规指标变化,比较两组患者VEGF、sTREM-1、PGE2水平,采用受试者工作特征(ROC)曲线分析VEGF、sTREM-1、PGE2单独及联合检测对直肠癌术后患者肛周感染的预测价值。结果与对照组相比,研究组红细胞平均体积、血红蛋白含量、平均血红蛋白量、血清VEGF水平降低,血清sTREM-1、PGE2水平升高(P<0.05);VEGF、sTREM-1、PGE2联合检测对直肠癌术后患者肛周感染诊断的曲线下面积(AUC)值高于单独检测的AUC值(P<0.05),且联合检测的敏感度为85.00%,特异度为93.90%。结论直肠癌术后患者肛周感染血常规部分指标发生下降,血清中VEGF水平降低,sTREM-1、PGE2水平升高,VEGF、sTREM-1、PGE2三者联合检测对直肠癌术后患者肛周感染具有较高的预测价值。OBJECTIVE To investigate the variation in blood routine and the predictive value of vascular endothelial growth factor(VEGF),soluble myeloid cell trigger receptor 1(sTREM-1)and prostaglandin E2(PGE2)in pa-tients with perianal infection after rectal cancer surgery.METHODS The clinical data of 60 patients with perianal infection after rectal cancer surgery in Hebei Hospital of Traditional Chinese Medicine from Feb 2019 to Feb 2023 were retrospectively collected as the observation group,and 82 patients with no perianal infection after rectal canc-er surgery during the same period were included in the control group.The clinical data between the two groups were statistically analyzed,the changes of blood routine indexes and the levels of VEGF,sTREM-1 and PGE2 were compared.The predictive value of VEGF,sTREM-1 and PGE2 individual and combined detection in postop-erative rectal cancer patients with perianal infection were analyzed by receiver operating characteristic curve(ROC).RESULTS Compared with the control group,the mean red blood cell volume,hemoglobin content,mean hemoglobin content and the level of serum VEGF in the observation group decreased,while the levels of serum sTREM-1 and PGE2 increased(P<0.05).The AUC value of the combined detection of VEGF,sTREM-1 and PGE2 in the diagnosis of postoperative rectal cancer patients with perianal infection was higher than that of the in-dividual detection(P<0.05),and the sensitivity and specificity of the combined detection were 85.00%and 93.90%,respectively.CONCLUSION Some blood routine indexes in postoperative rectal cancer patients with peri-anal infection decreased.The levels of serum VEGF decreased while sTREM-1 and PGE2 increased.The combined detection of VEGF,sTREM-1 and PGE2 had high predictive value for postoperative rectal cancer patients with perianal infection.
