摘  要:慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)是一种呼吸道慢性炎症性疾病,高发病率和死亡率造成了巨大社会和经济负担,2019年WHO公布COPD已成为全球第三大死亡原因[1]。但COPD的发病机制尚不明了。随着脂质代谢研究的深入,在COPD中肺泡巨噬细胞(alveolar macrophage,AM)内的脂质代谢变化也备受关注。Fujii等[2]在研究中发现了AM中的脂质组成以COPD疾病等级依赖性的方式发生变化,这些变化的脂质主要是鞘脂、胆固醇、磷脂和脂肪酸,表明脂质代谢在COPD病程的发生发展中起到一定的作用。The principle function of alveolar macrophage(AM)is to phagocytize and clear foreign antigens,which is the first line of defense against various invading pathogenic bacteria in mammalian lungs.AM has diversified functions mainly including antigen presentation and immune regulation,and it also plays pivotal roles in maintaining the homeostasis of the pulmonary immune system and is highly involved in the pathogenesis of chronic obstructive pulmonary disease(COPD).In-depth investigations have demonstrated that functions of AM are regulated by lipid metabolism,which participates in metabolic reprogramming in COPD,and thus inducing functional phenotypes.The present review mainly summarizes lipid metabolism change in AM,and its effects on inflammation,as well as the clinical applications in COPD.
