热敏灸联合穴位按摩治疗退行性膝关节炎临床观察

摘　　要：目的观察热敏灸联合穴位按摩治疗退行性膝关节炎(DKA)效果。方法将江西中医药大学附属医院2021年4月—2022年3月收治的60例DKA患者随机分为两组,对照组采用西药外敷,试验组采用热敏灸联合穴位按摩。比较两组疼痛和频率双重量表(2D-VAS)、健康调查简表(SF-36)评分和总有效率,检测血清透明质酸(HA)、基质金属蛋白酶-9(MMP-9)、白细胞介素-1β(IL-1β)等炎性因子水平。结果试验组总有效率83.33%(25/30)优于对照组的53.33%(16/30)(P<0.05);两组在疗程结束后和随访时疼痛程度、发作频率均下降(P<0.01),且试验组2D-VAS评分较对照组更低(P<0.01);两组在疗程结束后和随访时躯体状况和精神状况均有改善(P<0.05),试验组SF-36评分更高(P<0.05);治疗后两组HA、MMP-9、IL-1β均下降(P<0.05),试验组低于对照组(P<0.05)。结论热敏灸联合穴位按摩治疗DKA能够减轻患者疼痛程度和减缓发作频率、改善其炎性病理状态。Objective To observe the efficacy of heat-sensitive moxibustion combined with acupoint massage in the treatment of de⁃generative knee arthritis(DKA).Methods A total of 60 patients with DKA in Affiliated Hospital of Jiangxi University of Chinese Medi⁃cine from April 2021 to March 2022 were randomly divided into control group treated with external application of Western medicine and experimental group treated with heat-sensitive moxibustion and acupoint massage.The pain and frequency scale(2D-VAS),health questionnaire(SF-36)and response rate were compared.Serum hyaluronic acid(HA),matrix metalloproteinase 9(MMP-9)and interleukin-1β(IL-1β)were collected.Results The total effective rate of experimental group was 83.33%(25/30)compared with that of control group(53.33%,16/30)(P<0.05).The pain degree and attack frequency of both groups were decreased after treatment and at follow-up(P<0.01),and the 2D-VAS score of experimental group was lower than that of control group(P<0.01).The physical condi⁃tion and mental condition of both groups were improved after treatment and at follow-up(P<0.05),and the SF-36 score of experimen⁃tal group was higher(P<0.05).After treatment,HA,MMP-9 and IL-1βwere decreased in both groups(P<0.05),and the experimental group was lower than the control group(P<0.05).Conclusion Heat-sensitive moxibustion combined with acupoint massage in the treatment of DKA can reduce the degree of pain and attack frequency,and improve the inflammatory pathological state.
