摘　　要：目的分析小儿过敏性紫癜在常规方法治疗的基础上应用冲击治疗(甲泼尼龙琥珀酸钠)的临床效果。方法选取2021年1月至2023年4月收治的小儿过敏性紫癜患儿60例,采用随机数字表法分为对照组和观察组,各30例。对照组采用常规方法治疗,观察组在对照组基础上联合应用冲击治疗(甲泼尼龙琥珀酸钠)。比较两组临床症状缓解时间、住院时间、临床疗效、免疫学指标、炎症反应指标变化及不良反应情况。结果治疗后,观察组总有效率高于对照组(P<0.05);观察组紫癜消退时间、住院时间、关节疼痛缓解时间、水肿消失时间、消化道症状缓解时间短于对照组(P<0.05);两组外周血CD4^(+)、CD3^(+)、CD4^(+)/CD8^(+)水平表现为升高趋势,且观察组水平更高(P<0.05);两组血清免疫球蛋白A(Ig A)、肿瘤坏死因子-α(TNF-α)、免疫球蛋白M(IgM)、白介素-6(IL-6)、免疫球蛋白E(Ig E)、白介素-10(IL-10)、补体C3、外周血CD8^(+)水平降低,且观察组水平更低(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论过敏性紫癜患儿应用冲击治疗(甲泼尼龙琥珀酸钠),可有助于改善其免疫功能,缓解临床症状,下调炎症介质水平,进而提高临床治疗效果,且不增加不良反应。Objective To analyze the effects of impact therapy(methylprednisolone sodium succinate)in children with anaphylactoid purpura on the basis of conventional treatment.Methods 60 children with anaphylactoid purpura admitted from January 2021 to April 2023 were divided into control group and observation group by random number table method,with 30 cases in each group.The control group was treated with conventional methods,and the observation group was combined with impact therapy(methylprednisolone sodium succinate)on the basis of the control group.The time of clinical symptom resolution,hospital stay,clinical efficacy,immunological indexes,changes of inflammatory response indexes,and adverse reactions were compared between the two groups.Results After treatment,the total effective rate of observation group was higher than that of the control group(P<0.05).In the observation group,the time of purpura resolution,hospital stay,joint pain relief time,edema disappearance time and gastrointestinal symptoms relief time were shorter than those of the control group(P<0.05).The levels of CD4^(+),CD3^(+)and CD4^(+)/CD8^(+)in peripheral blood of the two groups showed an increasing trend,and the levels of the observation group were higher(P<0.05).The levels of serum immunoglobulin A(IgA),tumor necrosis factor-α(TNF-α),immunoglobulin M(IgM),interleukin-6(IL-6),immunoglobulin E(IgE),interleukin-10(IL-10),complement C3 and peripheral blood CD8^(+)in both groups were decreased,and the levels of observation group were lower(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The application of impact therapy(methylprednisolone sodium succinate)in children with anaphylactoid purpura can help improve their immune function,relieve clinical symptoms,reduce the level of inflammatory mediators,and then improve clinical treatment effectiveness without increasing adverse reactions.
