摘  要:目的:探讨强化爬行训练对于脑干卒中后平衡功能障碍的治疗效果。方法:选择2019年5月~2020年5月柳州市中医医院康复医学科收治的50例脑干卒中后平衡功能障碍的患者。根据随机数字表将受试者为观察组和对照组各25例。对照组采用常规康复治疗手段,观察组则在常规康复治疗的基础上加上爬行训练。采用10米步行试验(10WMT)、Holden步行能力分级(FAC)、Berg平衡量表(BBS)以及功能独立性评定(FIM)量表进行评价,比较两组治疗效果。结果:治疗8周后,观察组和对照组的Holden步行能力分级、BBS、FIM、10WMT评分均高于治疗前(P<0.05);观察组的各项测试评分皆明显优于对照组的评分(P<0.05)。结论:爬行训练有助于改善脑干卒中后平衡功能障碍,通过使用爬行设备可以使患者很好参与康复各个阶段,从而提高患者稳定性、活动性,改善患者当前的生活质量,具有积极的作用,值得推广。Objective:To explore the therapeutic effect of intensive crawling training on balance dysfunction after brain stem stroke.Methods:50 patients with balance dysfunction after brain stem stroke admitted to the Department of Rehabilitation Medicine,Liuzhou Hospital of Traditional Chinese Medicine from May 2019 to May 2020 were selected.Subjects were included in the experimental and control groups according to the random number table,with 25 patients in each group.The control group used conventional rehabilitation treatment methods,and the experimental group added crawling training on the basis of conventional rehabilitation treatment.The 10-meter walk test(10WMT),Holden's Walking Ability Classification(FAC),Berg Balance Scale(BBS),and Functional Independence Rating(FIM)scale were used to evaluate and compare the treatment effects of the two groups.Results:After 8 weeks of treatment,the Holden Walking Ability Classification,BBS,FIM,and 10WMT scores of the observation and control groups were higher than those before treatment(P<0.05);the scores of all tests in the observation group were significantly better than those in the control group(P<0.05).Conclusions:Crawling training helps to improve balance dysfunction after brainstem stroke,and the use of crawling equipment allows patients to participate well in all phases of rehabilitation,thus improving stability,mobility,and the current quality of life of patients,which has a positive effect and deserves to be promoted.
