五酯胶囊联合熊去氧胆酸对老年慢性乙型肝炎患者的疗效及免疫炎症反应的影响

摘　　要：目的:探讨五酯胶囊联合熊去氧胆酸胶囊对老年慢性乙型肝炎患者的治疗效果及对免疫炎症反应的影响。方法:研究对象为2020年10月—2022年10月在常州市第三人民医院就诊的136例老年慢性乙型肝炎患者,随机分为两组,各68例。对照组予以熊去氧胆酸胶囊治疗,观察组则在对照组基础上增加五酯胶囊治疗。两组患者均接受为期3个月的治疗,并进行随访观察。对比两组临床疗效及治疗方案的安全性;检测并对比两组治疗前后血清丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、γ-谷氨酰转移酶(GGT)、胆碱酯酶(ChE)、前清蛋白(PAB)、C反应蛋白(CRP)及免疫球蛋白G(IgG)水平。结果:相较于对照组(88.24%),观察组总有效率(97.06%)更高(P<0.05);治疗后,两组血清ALT、AST、GGT、CRP和IgG水平均有所降低,观察组较对照组均更低,ChE和PAB水平均有所提高,观察组较对照组均更高,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:联合应用五酯胶囊和熊去氧胆酸胶囊在老年慢性乙型肝炎患者中有显著疗效,能促进肝功能的恢复,并显著减轻体免疫炎症反应,且治疗安全性高。Objective:To explore the therapeutic effect of Wuzhi Capsules combined with Ursodeoxycholic Acid Capsules on elderly patients with chronic hepatitis B and its influence on immune inflammatory reaction.Method:The study subjects were 136 elderly patients with chronic hepatitis B who were treated in Changzhou Third People's Hospital from October 2020 to October 2022,and they were randomly divided into two groups,68 cases in each group.The control group was treated with Ursodeoxycholic Acid Capsules,and the observation group was treated with Wuzhi Capsules on the basis of the control group.Patients in both groups were treated for 3 months and followed up.The clinical efficacy and the safety of the treatment plan were compared between the two groups.Serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),γ-glutamyltransferase(GGT),choline esterase(ChE),prealbumin(PAB),C reactive protein(CRP)and immunoglobulin G(IgG)levels were detected and compared between the two groups before and after treatment.Result:Compared with the control group(88.24%),the total effective rate(97.06%)of the observation group was higher(P<0.05).After treatment,the levels of serum ALT,AST,GGT,CRP and IgG in the two groups were all decreased,those in the observation group were lower than those in the control group,and the levels of ChE and PAB in the two groups were all increased,those in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The combined application of Wuzhi Capsules and Ursodeoxycholic Acid Capsules has a significant effect in elderly patients with chronic hepatitis B,which can promote the recovery of liver function and significantly reduce the immune inflammatory reaction,and the treatment safety is high.
