小儿扁桃体切除术围术期镇痛研究进展

摘　　要：扁桃体切除术作为常见的小儿外科手术,在解决呼吸道问题方面取得显著效果,但术后的疼痛管理一直是需要深入关注的重要问题。术后疼痛可能引发一系列不良反应,如苏醒期躁动和术后出血,直接影响患儿术后恢复和身心健康。为提高术后恢复质量,目前已采用多种围术期疼痛管理措施,包括阿片类药物、对乙酰氨基酚、非甾体抗炎药、地塞米松、氯胺酮、扁桃体周围的局部浸润麻醉或表面麻醉以及补充替代疗法等。本文就应用于小儿扁桃体切除术的各类镇痛药物和镇痛方法的研究现状进行综述,为小儿扁桃体切除术围术期疼痛的个体化管理提供一定的理论依据。As a common pediatric surgery,tonsillectomy has achieved significant results in solving respiratory problems,but postoperative pain management has been an important issue that needs in-depth attention.Postoperative pain may cause a series of adverse reactions,such as restlessness during recovery and postoperative bleeding,which directly affect the postoperative recovery and physical and mental health of children.In order to improve the quality of recovery after surgery,a variety of perioperative pain management measures have been adopted,including opioids,acetaminophen,non-steroidal anti-inflammatory drugs,dexamethasone,ketamine,local infiltration anesthesia or surface anesthesia around tonsils,as well as complementary alternative therapies.This article reviews the research status of various analgesic drugs and methods used in pediatric tonsillectomy,and provides a theoretical basis for the individualized management of perioperative pain in pediatric tonsillectomy.
