摘　　要：目的探讨右美托咪定滴鼻在小儿上肢骨折手术麻醉前的应用效果。方法选取2020年3月至2022年5月济南市章丘区中医医院收治的64例上肢骨折手术麻醉患儿作为研究对象,以抛币法分为两组,每组32例。参照组采用咪达唑仑麻醉,研究组采用右美托咪定麻醉,比较两组的麻醉效果。结果两组给药后10 min(T1)、给药后20 min(T2)、给药后30 min(T3)的呼吸频率(RR)、心率(HR)以及血氧饱和度(SpO_(2))均低于给药前(P<0.05),但两组T1、T2、T3的RR、HR、SpO_(2)比较,差异无统计学意义(P>0.05);研究组躁动发生率以及呼吸道不良事件总发生率低于参照组,差异有统计学意义(P<0.05);研究组Ramsay评分高于参照组,差异有统计学意义(P<0.05);研究组儿童医院疼痛量表评分高于参照组,差异有统计学意义(P<0.05)。结论右美托咪定滴鼻在小儿上肢骨折手术麻醉前的应用效果显著,能够减少不良事件,改善麻醉效果,值得推广。Objective To investigate the application effect of Dexmedetomidine nasal drops before anesthesia in upper limb fracture surgery in children.Methods A total of 64 children who underwent upper limb fracture surgery in Jinan Zhangqiu District Hospital of Traditional Chinese Medicine from March 2020 to May 2022 were selected and divided into two groups by the coin tossing method,with 32 cases in each group.The reference group was anesthetized with Midazolam,and the study group was anesthetized with Dexmedetomidine.The anesthetic effect of the two groups was compared.Results The respiratory rate(RR),heart rate(HR)and oxygen saturation(SpO_(2))of 10 min(T1),20 min(T2)and 30 min(T3)after administration were all lower than those before administration(P<0.05),but there were no s ignificant differences in RR,H R and SpO_(2) at T1,T2 and T3 between the two groups(P>0.05).The incidence of agitation and the overall incidence of respiratory adverse events in the study group were lower than those in the reference group(P<0.05).The Ramsay score in the study group was higher than that in the reference group,the difference was statistically significant(P<0.05).The children’s hospital of Eastern Ontario pain scale score in the study group was higher than that in the reference group,the difference was statistically significant(P<0.05).Conclusion Dexmedetomidine nasal drops before anesthesia in upper limb fracture surgery in children has remarkable application effect,which can reduce adverse events and improve the anesthe tic effect,and is worth promoting.
