摘　　要：目的探讨EB病毒相关噬血细胞性淋巴组织细胞增生症(EBV-HLH)患者细胞因子水平的变化。方法回顾性病例对照研究。回顾性分析2022年2月至2023年2月华中科技大学同济医学院附属同济医院收治的65例EBV-HLH、30例传染性单核细胞增多症(IM)、40例其他感染相关的HLH患者临床资料。采用酶联免疫吸附试验(ELISA)检测血清标本中白细胞介素(IL)-6、IL-2、IL-10、IL-8、IL-1β、肿瘤坏死因子α(TNF-α)、干扰素γ(IFN-γ)水平,分别比较EBV-HLH组与IM组、其他感染相关的HLH组的细胞因子水平。结果EBV-HLH组IL-6、IL-2、IL-10、IL-8、IL-1β、TNF-α、IFN-γ等细胞因子水平均高于其他感染相关的HLH组,差异均有统计学意义(均P<0.05);EBV-HLH组IL-2、IL-10、IFN-γ细胞因子水平均高于IM组,差异均有统计学意义(均P<0.05)。结论EBV-HLH患者IL-6、IL-2、IL-10、IL-8、IL-1β、TNF-α、IFN-γ细胞因子水平均升高,可能在EBV-HLH发生、发展过程中发挥作用。Objective To investigate the level change of cytokines in patients with Epstein-Barr virus(EBV)-associated hemophagocytic lymphohistiocytosis(EBV-HLH).Methods A retrospective case control study was conducted.The clinical data of 65 patients with EBV-HLH,30 patients with infectious mononucleosis(IM)(IM group)and 40 patients with non-EBV infection-associated hemophagocytic lymphohistiocytosis(non-EBV-HLH group)who admitted to Tongji Hospital,Tongji Medical College of Huazhong University of Science and Technology from February 2022 to February 2023 were retrospectively analyzed.The enzyme-linked immunosorbent assay(ELISA)was used to detect the expression levels of the interleukin(IL)-6,IL-2,IL-10,IL-8,IL-1β,tumor necrosis factor-α(TNF-α)and interferonγ(IFN-γ)in serum samples of patients in the above 3 groups.The cytokines levels in EBV-HLH group were compared with those in IM group and non-EBV-HLH group,respectively.Results The cytokines levels of IL-6,IL-2,IL-10,IL-8,IL-1β,TNF-αand IFN-γin EBV-HLH group were higher than those in the non-EBV-HLH group,and the differences were statistically significant(all P<0.05).The cytokines levels of IL-2,IL-10 and IFN-γin EBV-HLH group were higher than those in IM group,and the differences were statistically significant(all P<0.05).Conclusions The cytokines levels of IL-6,IL-2,IL-10,IL-8,IL-1β,TNF-α,IFN-γare increased in EBV-HLH patients,which may play an important role in the development and progression of EBV-HLH.
