摘 要:机械循环支持(MCS)已成为心脏移植过渡期的首选治疗方法, 并有望成为终末期心力衰竭的长期治疗方案。心室辅助装置(VAD)是目前最常用的MCS设备之一, 然而适用于小儿的循环辅助装置却非常有限, 在罹患终末期心衰的儿童尤其是婴幼儿中, 应用VAD进行循环辅助治疗仍然存在巨大挑战, 装置的植入时间、泵的选择和术后并发症控制等方面仍然需要进一步研究和改进。本文综述儿童VAD的应用、主要并发症、术后管理和未来挑战, 供临床参考。Mechanical circulatory support(MCS)has become the treatment of choice in the transition to heart transplantation,and is expected to become a long-term treatment option for end-stage heart failure.Ventricular assist device(VAD)is one of the most commonly used MCS devices,however,circulatory assist devices for paediatric patients are very limited,and the application of VAD for circulatory assist in children with end-stage heart failure,especially infants and children,still poses significant challenges,for example,the timing of device implantation,pump selection,and control of postoperative complications still need to be further studied and improved.In the future,with the continuous progress and development of science and technology,more suitable VAD for children will be introduced,and its clinical application will become more extensive,and the survival rate and quality of life of children with heart failure will be further improved.
