摘 要:目的分析床旁颅脑超声在早产儿颅内出血中的诊断价值,着重探讨不同胎龄和不同出生体质量对早产儿颅内出血发生率产生的影响。方法按照随机数字表法,抽选96例濮阳市中医医院超声介入科接收的早产儿,抽选时间是2021年6月-2022年6月,分析早产儿颅内出血的主要超声表现,并观察床旁颅脑超声对不同胎龄、体重早产儿颅内出血的发生情况。结果共检出26例颅内出血,发生率为27.08%;出血部位:脑室内出血占88.46%,脑实质出血占7.69%,硬脑膜下出血占3.85%;男早产儿与女早产儿的性别无差异(P>0.05);胎龄<32周的早产儿颅内出血率39.13%明显高于胎龄≥32周的颅内出血率16.00%(P<0.05);出生体质量<1500 g的早产儿颅内出血率38.09%高于出生体质量≥1500 g的早产儿颅内出血率18.52%(P<0.05)。结论低胎龄、低体重的早产儿发生颅内出血概率更高;予以早产儿颅内出血行床旁颅脑超声检查,具有无创、便捷的优点,可获取准确、可靠的信息,可将其作为筛查、监测早产儿颅内出血的重要手段,值得临床推广。Objective To analyze the diagnostic value of bedside cranial ultrasound in preterm infants with intracranial hemorrhage,and to explore the effect of different gestational ages and birth weight of preterm infants on the incidence of intracranial hemorrhage.Methods According to random number table method,96 cases of premature infants admitted in department of ultrasonic intervention of Puyang Traditional Chinese Medicine hospital from June 2021 to June 2022 were selected into this study.The main ultrasonic manifestations of intracranial hemorrhage in premature infants were analyzed,and the incidence of intracranial hemorrhage in premature infants with different gestational age and weight was observed by bedside brain ultrasound.Results 26 cases of intracranial hemorrhage were detected,and the incidence was 27.08%.There was intraventricular hemorrhage accounting for 88.46%.Cerebral parenchymal hemorrhage accounted for 7.69%,and subdural hemorrhage was 3.85%.There was no gender difference between male and female preterm infants(P>0.05).The rate of intracranial hemorrhage in preterm infants with gestational age<32 weeks was 39.13%,which was significantly higher than that in preterm infants with gestational age≥32 weeks,16.00%(P<0.05).The intracranial hemorrhage rate of preterm infants with birth weight<1500g was 38.09%which was higher than that of preterm infants with birth weight≥1500g(18.52%,P<0.05).Conclusion Premature infants with low gestational age and low body weight have a higher probability of intracranial hemorrhage.Bedside cranial ultrasound examination for intracranial hemorrhage in preterm infants is noninvasive and convenient,and it can obtain accurate and reliable information.It can be used as an important method to screen and monitor intracranial hemorrhage in preterm infants,which is worthy of clinical promotion.
