摘 要:目的探讨适用于超声下静脉穿刺的耐高压留置针穿刺方法的设计与应用。方法按随机数字表法将2019年1月-2022年5月于南昌大学第二附属医院行CT检查且静脉穿刺困难的100例患者分为对照组和观察组,每组均为50例。对照组采用超声下Y型留置针穿刺方法进行置管,观察组采用改良留置针穿刺方法进行置管。对两组患者一次性穿刺成功率、穿刺情况、并发症发生情况进行评价和比较。结果观察组一次性穿刺成功率(96.00%)高于对照组(84.00%)(P<0.05)。观察组穿刺次数少于对照组,穿刺时间短于对照组,留针时间长于对照组(P<0.05)。观察组导管异位、误穿动脉、静脉炎等并发症总发生率为4.00%(2/50),低于对照组的18.00%(9/50)(P<0.05)。结论设计适用于超声下静脉穿刺的耐高压留置针,并对留置针穿刺方法进行改良和优化后,一次性穿刺成功率大幅提升,穿刺时间也有所缩短,有助于提升静脉留置针留置效果,静脉炎、导管异位等并发症较少,可积极推广。Objective To explore the design and application of high pressure indwelling puncture method suitable for ultrasonic venipuncture.Methods A total of 100 patients who underwent CT examination in our hospital from January 2019 to May 2022 were divided into control group and observation group according to random number table method,with 50 patients in each group.The control group was implanted with Y-shaped indwelling needle under ultrasound,and the observation group was implanted with improved indwelling needle puncture.The one-time puncture success rate,puncture situation and complications of the two groups were evaluated and compared.Results The success rate of one-time puncture in observation group(96.00%)was higher than that in control group(84.00%)(P<0.05).The puncture times,puncture time and retention time of observation group were shorter than control group(P<0.05).The total incidence of complications such as catheterization,arterial puncture and phlebitis in the observation group was 4.00%(2/50),which was lower than that in the control group(18.00%,9/50)(P<0.05).Conclusion The design of high pressure resistant indentation needle suitable for ultrasonic venipentesis,and the improvement and optimization of indentation needle puncture method,one-time puncture success rate is greatly improved,puncture time is also shortened,which is helpful to improve the indentation effect of venous indentation needle,and there are fewer complications such as phlebitis and catheter ectopic,which can be actively promoted.
