某基层社区卫生服务中心中医师处方存在的问题及分析

摘　　要：目的了解某社区卫生服务中心(以下简称“某中心”)中药饮片处方合理用药情况,为加强中药饮片处方管理和提高社区中药饮片处方质量提供依据。方法随机抽取某中心2023年每月任意100张的中药饮片处方,对饮片处方进行回顾性点评与分析。结果2023年共抽查中药饮片处方1200张,不合理处方160张,处方不合理率13.33%;其中,不规范处方139张(占不合理处方的86.88%),包括开具的处方缺少中医证型、未按规定注明煎煮等特殊要求及无特殊情况延长处方用量且未标明理由;用药不适宜处方26张(占不合理处方的16.25%),包括毒性饮片超量和联合用药不适宜且未再次签名盖章。结论某中心中药饮片处方存在一定的不规范情况,主要表现在脚注遗漏和无特殊情况延长处方用量。处方合格率有待进一步提高。通过处方点评,对处方中存在的问题及时进行合理干预,利于处方质量的提高,促进社区合理用药水平。Objective To understand the rational use of Traditional Chinese Medicine prescriptions in a community health service center(hereinafter referred to as"the center"),and to provide a basis for strengthening the management of Traditional Chinese Medicine prescriptions and improving the quality of community Traditional Chinese Medicine prescriptions.Method Randomly select 100 prescriptions of Traditional Chinese Medicine decoction pieces from a certain center every month in 2023,and conduct a retrospective review and analysis of the prescriptions.Result In 2023,a total of 1200 prescriptions for Traditional Chinese Medicine decoction pieces were randomly selected,including 160 unreasonable prescriptions,with an unreasonable prescription rate of 13.33%;among them,139 nonstandard prescriptions were issued(accounting for 86.88% of unreasonable prescriptions),including prescriptions that did not include Traditional Chinese Medicine syndrome types,did not indicate special requirements such as decoction as required,and extended prescription dosage without specifying reasons under no special circumstances;26 inappropriate prescriptions for medication(accounting for 16.25% of unreasonable prescriptions),including overdose of toxic decoction pieces and inappropriate combination therapy without re-signing and re-stamping.Conclusion There are certain irregularities in the prescription of Traditional Chinese Medicine decoction pieces in a certain center,mainly manifested in missing footnotes and extending the prescription dosage without special circumstances.The prescription qualification rate needs to be further improved.Through prescription review,timely and reasonable intervention should be carried out to address the problems in the prescription,which is beneficial for improving the quality of the prescription and promoting the level of rational drug use in the community.
