摘　　要：小金丹载于清代《外科证治全生集》,历来被认为是治疗阴疽类病证的代表方剂。目前学界多认为该方为王维德所创。笔者经文献考证发现,小金丹并非王维德原创。文章从方源考证、方名考辨、主治演变三方面对小金丹进行源流考证,结论如下:小金丹源于金元医家刘完素所创“一粒金丹”;在一粒金丹的流传过程中,“捉虎丹”是其别名,“赤龙丸”是明代沈之问将一粒金丹应用于麻风类疾病的加减方;一粒金丹的主治包括顽痹与阴疽类病证,加减为赤龙丸后可治麻风类疾病。结合刘完素玄府气液学说解析组方构架,一粒金丹融辛温开玄、香窜开玄、霸药溃坚等多种治法,能畅达玄府,祛除顽邪,对于顽痹以及乳腺增生症等疾病有异病同治的良好效果。文章旨在正本澄源,利于深入把握该方内涵,更好地服务于临床。Recorded in the Life-Saving Manual of Diagnosis and Treatment of External Diseases,Xiaojin Pills is constantly considered as a typical formula to treat dorsal furuncle.Nowadays the majority of scholars hold the thought that Xiaojin Pills was created by WANG Weide.Throughout the literature review,we found that Xiaojin Pills was not originally created by WANG Weide.After clarifying the original source of Xiaojin Pills and conducting relevant research on its name and indication evolution process,we obtained some conclusions about the origin and development of Xiaojin Pills.The conclusions are as follows:Xiaojin Pills was originated from'a grain of Xiaojin Pills',created by LIU Wansu in the Jin and Yuan dynasties;As it was handed down,a grain of Xiaojin Pills was also known as‘Zhuo Hu Pills'.SHEN Zhiwen in the Ming dynasty,used modified'a grain of Xiaojin Pills'to treat leprosy and named it'Chi Long Pills';A grain of Xiaojin Pills is mainly applied in dorsal furuncle and obstinate arthralgia.Also,it can be used to treat leprosy after adjusting its component.Combined with LIU Wansu's Xuanfu-Qi-Fluid theory,the formula framework was analyzed.A grain of Xiaojin Pills has a good effect on treating different diseases such as stubborn arthralgia and breast hyperplasia because of containing lots of Xuanfu-opening therapies such as acrid-warm opening orifices,aromatic medicinal unblocking channel,drastic medicinal promoting suppuration,which can unblock Xuanfu smoothly and eliminate obstinate evils.This article aims to clarify the source,which is helpful to deeply grasp the connotation of Xiaojin Pills and better apply it to clinical practice.
