摘　　要：目的:分析丝线重建颈后方韧带复合体联合Centerpiece支撑钢板在颈后路单开门椎管成形术中的应用价值。方法:选取2021年6月—2023年6月我院收治的80例多节段脊髓型颈椎病患者作为研究对象,采用随机数字表法将患者分为对照组(n=40)和观察组(n=40),对照组实施传统颈后路单开门缝线悬吊门轴干预;观察组实施丝线重建颈后方韧带复合体联合Centerpiece支撑钢板干预。比较两组患者神经功能、影像学观测指标(椎管矢状径、颈椎曲度、C_(2)~C_(7)Cobb角、颈椎活动度、最窄节段椎管面积)。结果:术后3个月~1年,观察组JOA评分高于对照组(P<0.05)。术后3个月~1年,观察组椎管矢状径、颈椎曲度、C_(2)~C_(7)Cobb角、颈椎活动度、最窄节段椎管面积均优于对照组(P<0.05)。术后3个月观察组C_(4)、C_(6)门轴骨愈合情况优于对照组(P<0.05),术后1年,两组患者C_(4)、C_(6)门轴骨愈合情况比较无差异(P>0.05)。结论:丝线重建颈后方韧带复合体联合Centerpiece支撑钢板应用于颈后路单开门椎管成形术中具备良好的治疗效果,能有效恢复颈椎功能,减轻对最窄节段椎管面积、颈椎活动度、门轴骨愈合情况及椎管扩大程度的影响,又能在一定程度上减少颈椎轴性症状的发生。Objective:To analyze the application value of silk thread reconstruction of posterior cervical ligament complex combined with Centerpiece support plate in posterior cervical single door laminoplasty.Methods:80 patients with multi segment cervical spondylotic myelopathy admitted to our hospital from June 2021 to June 2023 were selected as the research subjects.The patients were randomly divided into a control group(n=40)and an observation group(n=40)using a random number table method.The control group received traditional posterior cervical single door suture suspension door axis intervention.The observation group implemented silk thread reconstruction of the posterior cervical ligament complex combined with Centerpiece support plate intervention.Compare the neurological function and imaging observation indicators(sagittal diameter of the spinal canal,curvature of the cervical spine,C_(2)~C_(7) Cobb angle,cervical mobility,and narrowest segment spinal canal area)between the two groups of patients.Results:After 3 months to 1 year of surgery,the JOA score in the observation group was higher than that in the control group(P<0.05).After 3 months to 1 year of surgery,the observation group had better sagittal diameter,cervical curvature,C_(2)~C_(7) Cobb angle,cervical mobility,and narrowest segment spinal canal area than the control group(P<0.05).After 3 months of surgery,the observation group showed better healing of the C_(4),C_(6) portal axis compared to the control group(P<0.05).1 year after surgery,there was no difference in the healing of the C_(4),C_(6) portal axis between the two groups of patients(P>0.05).Conclusion:The application of silk thread reconstruction of the posterior cervical ligament complex combined with Centerpiece support plate in posterior cervical single door laminoplasty has a good therapeutic effect,which can effectively restore cervical function,reduce the impact on the narrowest segment spinal canal area,cervical mobility,portal axis bone healing,and spinal canal enlargement,and also reduce
