镇肝清心熄风汤治疗特应性皮炎临床观察

摘　　要：目的观察镇肝清心熄风汤治疗特应性皮炎临床疗效。方法选取2019年8月至2021年8月收治的64例特应性皮炎患者为研究对象,按照随机数字表法分为2组,对照组32例予西医常规治疗,治疗组32例在对照组基础上联合镇肝清心熄风汤治疗。2组均治疗2个月后统计疗效,比较2组治疗前后病情严重程度[采用特应性皮炎评分(SCORAD)、皮肤病生活质量指数(DLQI)和湿疹面积及严重程度指数(EASI)进行评价]、免疫功能指标[包括辅助性T淋巴细胞1(Th1)、Th2、Th17及调节性T淋巴细胞(Treg)]及炎症相关指标[包括白细胞介素10(IL-10)、转化生长因子β(TGF-β)、IL-17、IL-22及IL-23]水平变化情况。结果治疗组总有效率93.8%(30/32),对照组总有效率75.0%(24/32),治疗组总有效率高于对照组(P<0.05)。与本组治疗前比较,2组治疗后SCORAD评分、EASI评分及DLQI评分均降低(P<0.05),且治疗组治疗后SCORAD评分、EASI评分及DLQI评分均低于对照组(P<0.05)。与本组治疗前比较,2组治疗后免疫功能指标Th1及Treg水平均升高(P<0.05),Th2及Th17水平均降低(P<0.05),且治疗组治疗后Th1及Treg水平均高于对照组(P<0.05),Th2及Th17水平均低于对照组(P<0.05)。与本组治疗前比较,2组治疗后炎症相关指标IL-10、TGF-β、IL-22及IL-23水平均降低(P<0.05),且治疗组治疗后IL-10、TGF-β、IL-22及IL-23水平均低于对照组(P<0.05)。结论镇肝清心熄风汤治疗特应性皮炎效果良好,可有效促进皮损修复,缩小皮损面积,减轻皮损程度,提高生活质量,其作用机制可能与改善机体免疫环境,恢复免疫平衡,控制炎症因子的异常分泌,抑制炎症反应有关。Objective To observe the therapeutic effect of Zhengan Qingxin Xifeng Decoction in the treatment of atopic dermatitis(AD).Methods A total of 64 AD patients admitted to our hospital from August 2019 to August 2021 were enrolled,and they were randomized to control group(routine Western medicine[RWM],n=32)and treatment group(Zhengan Qingxin Xifeng Decoction plus RWM,n=32).After 2-month treatment,the aim was to compare the disease severity(including Scoring Atopic Dermatitis[SCORAD],dermatology life quality index[DLQI]and eczema area and severity index[EASI]),immune function indexes(including T helper-1[Th1],Th2,Th17 and regulatory T[Treg]),and inflammation-related indexes(including interleukin 10[IL-10],transforming growth factor-β[TGF-β],IL-17,IL-22 and IL-23)between the two groups before and after treatment.Results The total effective rate in the treatment group was significantly higher than that of the control group(93.8%[30/32]vs 75.00%[24/32],P<0.05).After treatment,patients in the both group had significantly reduced the scores of SCORAD,EASI and DLQI,significantly increased the levels Th1 and Treg,significantly decreased the levels of Th2 and Th17,significantly lower the levels of IL-10,TGF-β,IL-22 and IL-23 than those before treatment(all P<0.05),and the improvements were more pronounced in the treatment group than in the control group(P<0.05).Conclusion Zhengan Qingxin Xifeng Decoction is effective on AD patients.It can effectively promote the repair of skin lesions,reduce the area of skin lesions,reduce the degree of skin lesions,and improve the quality of life.It is capable of promoting the recovery of immune balance,controlling the abnormal secretion of inflammatory factors,and inhibiting the inflammatory response by improving immune environment.
