解毒祛瘀滋阴法联合针刺治疗系统性红斑狼疮的疗效

摘　　要：目的 观察解毒祛瘀滋阴法联合针刺治疗系统性红斑狼疮(SLE)的疗效。方法 选择2021年5月至2023年1月在驻马店市中心医院治疗的75例SLE患者为研究对象,按照随机数字表法将其分为针刺组(采用针刺治疗,n=37)和联合治疗组(采用解毒祛瘀滋阴法联合针刺治疗,n=38)。检测两组生化指标,包括白细胞介素-18(IL-18)、补体C3、25-羟基维生素D[25(OH)D]、白细胞介素-1β(IL-1β);评估两组甲襞微循环情况(微循环流态、形态及袢周积分)、SLE疾病活动指数(SLEDAI)情况、中医证候情况及临床疗效。结果 与针刺组比较,联合治疗组25(OH)D、补体C3表达量较高,IL-18、IL-1β表达量及中医证候评分、SLEDAI评分、甲襞微循环评分较低,差异有统计学意义(P<0.05)。联合治疗组患者总有效率较针刺组高,差异有统计学意义(P<0.05)。结论 解毒祛瘀滋阴法联合针刺治疗SLE可调节患者免疫功能,抑制炎症,改善甲襞微循环,提升其临床疗效。Objective To observe the therapeutic effect of traditional Chinese medicine(TCM)Jiedu Quyu Ziyin prescription(with the function of detoxication,removing stasis and nourishing Yin)combined with acupuncture on systemic lupus erythematosus(SLE).Methods A total of 75 SLE patients treated in Zhumadian Central Hospital from May 2021 to January 2023 were selected and divided into acupuncture group(acupuncture treatment,n=37)and combined treatment group(acupuncture treatment combined with oral administration of Jiedu Quyu Ziyin decoction,n=38)by random number table method.Biochemical markers in both groups were detected,including serum interleukin-18(IL-18),complement C3,25-hydroxyvitamin D[25(OH)D]and interleukin-1β(IL-1β).Nail fold microcirculation(microcirculation pattern,morphology,and loop integral),SLE disease activity index(SLEDAI),TCM syndromes and clinical efficacy were evaluated in both groups.Results Compared with the acupuncture group,the combined treatment group had higher levels of serum 25(OH)D and complement C3,but lower levels of serum IL-18 and IL-1β,TCM syndrome score,SLEDAI score and nail fold microcirculation score,and the differences were statistically significant(P<0.05).The total effective rate in the combined treatment group was higher than that in the acupuncture group,and the difference was statistically significant(P<0.05).Conclusion TCM Jiedu Quyu Ziyin decoction combined with acupuncture in the treatment of SLE can regulate immune function,inhibit inflammation,improve nail fold microcirculation,and enhance clinical efficacy.
