摘 要:目的 了解医院门诊预约后平均等待时间现状并探讨其影响因素,为门诊二次报到规则及门诊就医流程的设定提供参考依据。方法 通过医院信息系统(HIS)提取核工业四一六医院2021年10月至2022年9月门诊就诊相关数据,通过描述性统计分析和多元线性回归分析探讨门诊预约后平均等待时间的影响因素。根据影响因素提出并实施部分改进措施,再收集各门诊预约后的平均等待时间与实施改进措施前的门诊预约后平均等待时间进行比较。结果 患者预约方式、性别、年龄、职业、学历、就诊科室和医生职称的门诊预约后平均等待时间比较,差异有统计学意义(P<0.05)。对门诊预约后平均等待时间影响较大的是医生职称和预约方式。通过对门诊预约后平均等待时间影响因素、预约挂号和二次报到规则进行改进后,门诊预约后平均等待时间缩短1.41 min。结论 研究方法和结果分析为调整流程、缩短门诊预约后平均等待时间提供了有效的帮助;根据影响门诊预约后平均等待时间的相关因素推动预约制度落实、合理安排医生出诊、根据不同门诊特点设置差异化的二次报到规则等措施可能是缩短门诊预约后平均等待时间的有效方法。Objective To understand the current situation of average waiting time after outpatient appointments in hospitals and explore its influencing factors,so as to provide a basis for the second registration rules and outpatient medical process settings.Methods The data related to outpatient visits in Nuclear Industry 416 Hospital from October 2021 to September 2022 were extracted from hospital information system(HIS).Descriptive statistical analysis and multiple linear regression analysis were used to analyze the influencing factors of the average waiting time after outpatient appointments.Some improvement measures based on the influencing factors were proposed and implemented.And the average waiting time in outpatient clinics before and after the implementation of the improvement measures was compared and evaluated.Results There were statistically significant differences in the average waiting time after outpatient appointments among patients with different appointment method,gender,age,occupation,education background,doctor's professional title and visiting department(P>0.05).The factors that had the greatest impact on the average waiting time for outpatient services were the doctor's professional title,and appointment method.By improving the factors affecting the average waiting time after outpatient appointments,as well as the rules of appointment registration and secondary registration,the average waiting time for outpatient services was reduced by 1.41 min.Conclusion The research approach and result analysis provide effective assistance in adjusting processes and shortening average waiting time after outpatient appointments.Based on the analysis results,promoting the implementation of appointment systems and improving reasonable scheduling for doctors,and setting differentiated secondary registration rules according to different outpatient characteristics may be effective methods to shorten waiting time in outpatient clinics.
