摘  要:目的利用孟德尔随机化(Mendelian randomization,MR)方法探究肌肉减少症与心房颤动发病风险之间的因果关系,为老年人心房颤动的防治提供理论依据。方法基于全基因关联研究数据,以肌肉减少症的特征指标左手握力、右手握力和四肢肌肉质量(appendicular lean mass,ALM)为暴露因素,AF为结局,通过MR评估左手握力、右手握力和ALM对AF的因果效应。结果逆方差加权法(inverse variance weighted,IVW)结果显示,左手握力(OR=0.782、95%CI:0.6770.904、P<0.001)、右手握力(OR=0.646、95%CI:0.5580.748、P<0.001)和ALM(OR=0.769、95%CI:0.7390.799、P<0.001)与心房颤动的发病风险降低相关,且结果无异质性和多效性。结论左手握力、右手握力和ALM增加可能有助于预防和治疗心房颤动。Objective This study used mendelian randomization(MR)to investigate the causal relationship between sarcopenia and the risk of atrial fibrillation,providing a theoretical basis for the prevention and treatment of atrial fibrillation in elderly.Methods Based on the dataset from the Genome Wide Association Study(GWAS),the characteristic indicators of sarcopenia including left hand grip strength,right hand grip strength,and appendicular lean mass(ALM)were used as the exposure factors for sarcopenia,and AF were used as the outcome.The causal effects of left hand grip strength,right hand grip strength and ALM on AF were evaluated by MR.Results The results of the inverse variance weighted(IVW)analysis showed that left hand grip strength(OR=0.782,95%CI:0.6770.904,P<0.001),right hand grip strength(OR=0.646,95%CI:0.5580.748,P<0.001),and ALM(OR=0.769,95%CI:0.7390.799,P<0.001)were associated with a reduced risk of AF,and there was no heterogeneity or validity in the results.Conclusion An increase in left hand grip strength,right hand grip strength,and ALM may be useful for prevent and treat AF.
