苯扎氯铵溶液处置伤口后留置“优拓”引流条对动物致伤患者伤口愈合效果的影响

摘　　要：目的 探讨苯扎氯铵溶液处置伤口后留置“优拓”引流条对动物致伤患者伤口愈合效果的影响。方法 选取2020年1月—2022年12月于青岛市第五人民医院收治的动物致伤患者60例,根据随机数字表法分为两组,每组30例。对照组采用常规清创缝合术,观察组在清创缝合术后于伤口处留置“优拓”引流条,每日更换一次,直至拆线。比较两组患者的伤口愈合时间、瘢痕宽度、伤口感染率、引流情况、创口局部症状评分、创口炎症细胞计数、血清炎症细胞因子及炎症抑制因子水平。结果 观察组伤口愈合时间、瘢痕宽度均短于对照组,差异有统计学意义(P<0.01)。观察组伤口感染率低于对照组,差异有统计学意义(P<0.05)。观察组引流量少于对照组,引流持续时间短于对照组,差异有统计学意义(P<0.01)。治疗后7 d,两组疼痛评分、肿胀评分、瘙痒评分低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.01)。治疗后7 d,两组中性粒细胞、淋巴细胞、单核细胞和嗜酸性粒细胞计数低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.01)。治疗后7 d,两组白细胞介素(IL)-1β、IL-6、肿瘤坏死因子(TNF)-α水平低于治疗前,IL-10水平高于治疗前,且观察组IL-1β、IL-6、TNF-α水平低于对照组,IL-10水平高于对照组,差异有统计学意义(P<0.01)。结论 苯扎氯铵溶液和“优拓”引流条能够清洁伤口,消除细菌,减少渗出物,促进血液循环,加速组织修复,降低炎症反应,从而达到治愈动物致伤患者伤口的目的。Objective To investigate the influence of indwelling"Youtuo"drainage strips after treating wounds with benzalkonium chloride solution on wound healing effect of animal injured patients.Methods Sixty patients with animal injuries admitted to Qingdao Fifth People's Hospital from January 2020 to December 2022 were selected and divided into two groups by using a random number table method,with 30 cases in each group.Conventional debridement and suturing were used in control group,observation group retained a"Youtuo"drainage strip at the wound site and replaced it daily until the thread was removed after debridement and suturing.The wound healing time,scar width,wound infection rate,drainage situation,local symptom score of the wound,wound inflammatory cell count,serum levels of inflammatory cytokines and inhibitory factors between two groups were compared.Results The wound healing time and scar width in observation group were shorter than those in control group,and the differences were statistically significant(P<0.01).The wound infection ratein observation group was shorter than that in control group,and the difference was statistically significant(P<0.05).The drainage volume in observation group was less than that in control group,and the drainage duration in observation group was shorter than that in control group,and the differences were statistically significant(P<0.01).Seven days after treatment,the pain score,swelling score and itching score of two groups were lower than those before treatment,and observation group was lower than control group,and the differences were statistically significant(P<0.01).Seven days after treatment,the counts of neutrophils,lymphocytes,monocytes and eosinophils in two groups were lower than those before treatment,and observation group was lower than control group,and the differences were statistically significant(P<0.01).Seven days after treatment,the levels ofinterleukin(IL)-1β,IL-6,tumor necrosis factor(TNF)-αin two groups were lower than those before treatment,and IL-10 level
