基于“亢害承制”理论探讨巨噬细胞外泌体在慢性心力衰竭发展中的作用

摘　　要：巨噬细胞外泌体影响心脏炎症微环境,在慢性心力衰竭病程中发挥了关键作用。“亢害承制”理论是中医认识人体生克制化、病理失衡及指导辨证施治的重要法则。本文基于“亢害承制”理论,从理论依据、发病机制、病理生理联系及现代研究方面阐述巨噬细胞外泌体在慢性心力衰竭发展中的作用,提出运用“扶正调平,祛邪除亢”法治疗慢性心力衰竭,通过调控巨噬细胞外泌体减轻心脏炎症反应,以期为慢性心力衰竭的诊治提供新思路。Macrophage exosomes influence the cardiac inflammatory microenvironment and play a key role in the course of chronic heart failure.The theory of“harmful hyperactivity and responding inhibition”is an important rule for TCM to understand the body’s physiological restraint and pathological imbalance and to guide the syndrome differentiation and treatment.Based on the theory of“harmful hyperactivity and responding inhibition”,this article discussed the role of macrophage exosomes in the development of chronic heart failure from the aspects of theoretical basis,pathogenesis,pathophysiology and modern research.It was proposed to use the method of“supporting healthy qi to restore balance,dispelling pathogenic factors to remove hyperactivity”to treat chronic heart failure,and reduce cardiac inflammation by regulating macrophage exosomes,in order to provide a new reference for the diagnosis and treatment of chronic heart failure.
