成人急性复杂性阑尾炎术中腹腔灌洗与简单抽吸的效果比较

摘　　要：目的比较成人急性复杂性阑尾炎术中腹腔灌洗与简单抽吸的效果。方法收集本院141例行手术治疗的成人急性复杂性阑尾炎患者的临床资料,将患者分为腹腔灌洗组与简单抽吸组。结果两组患者术后首次排气时间、住院时间,术后3 d中性粒细胞与淋巴细胞比值(NLR)和C反应蛋白与白蛋白比值(CAR)、术后拔管时间差异无统计学意义(P>0.05);两组患者术后腹泻或腹胀发生情况差异无统计意义(P>0.05);两组患者术后体温峰值,术后体温>37.8℃差异有统计学意义(P<0.05)。结论急性复杂性阑尾炎患者术中行腹腔灌洗或简单抽吸不影响患者住院时间、术后胃肠道功能恢复及术后3 d炎症指标,但行腹腔灌洗的患者术后出现体温升高风险较高。Objective To investigate the effect of peritoneal lavage during operation of acute complicated appendicitis in adults,and to provide guidance for the surgical treatment of complicated appendicitis.Methods Patients with acute complex appendicitis who had undergone surgical treatment in our hospital were selected as the study objects.According to the surgical records,the patients were divided into"abdominal lavage"group and"simple suction"group.Results There were no significant differences in the first postoperative exhaust time,hospital stay,neutrophil to lymphocyte ratio(NLR),C-reactive protein to albumin ratio(CAR),and postoperative extubation days between the two groups(P>0.05).There was no significant difference in postoperative diarrhea or abdominal distension between the two groups(P>0.05).When the postoperative temperature peak value and postoperative temperature peak value were>37.8℃,the simple aspiration group was better than the peritoneal lavage group,and the difference was statistically significant(P<0.05).Conclusion In patients with acute complex appendicitis treated with surgery in our unit,intraoperative peritoneal lavage or simple aspiration did not affect the length of hospital stay,postoperative gastrointestinal function recovery,and postoperative inflammatory parameters at 3 days,but patients who underwent peritoneal lavage had a higher risk of postoperative hyperthermia.
