海桐皮汤外洗联合同种异体肌腱解剖重建外侧韧带治疗踝关节慢性不稳定的疗效

摘　　要：目的观察海桐皮汤外洗联合同种异体肌腱解剖重建外侧韧带治疗踝关节慢性不稳定的疗效。方法选取2020年12月至2022年12月同种异体肌腱解剖重建外侧韧带手术治疗患者42例,随机分为2组,对照组患者给予同种异体肌腱重建外侧肌腱及常规术后康复,观察组在对照组治疗基础上应用海桐皮外洗,比较两组术前、术后3个月VAS疼痛评分、踝与后足功能AOFAS评分、压力中心摆动距离、踝关节支撑期最大背伸角度。结果两组患者术后3个月的VAS疼痛评分、踝与后足功能AOFAS评分、压力中心摆动距离比较、踝关节支撑期最大背伸角度均优于术前(P<0.05);观察组术后3个月VAS疼痛评分、踝与后足功能AOFAS评分、踝关节支撑期最大背伸角度均优于对照组(P<0.05),观察组术后3个月压力中心摆动距离与对照组比较,差异无统计学意义(P>0.05)。结论海桐皮汤对同种异体肌腱解剖重建外侧韧带患者术后疼痛缓解、踝关节功能改善疗效显著,能加速患者康复。Objective To observe the effect of Haitongpi Decoction on postoperative rehabilitation of patients with chronic ankle instability treated by anatomical reconstruction of lateral ligament with allogeneic tendon.Methods From December 2020 to December 2022,42 patients who underwent anatomical reconstruction of lateral ligament with allogeneic tendon were divided into two groups randomly.The patients in the blank group completed the reconstruction of lateral tendon with allogeneic tendon and routine postoperative rehabilitation.The control group was treated with Haitongpi external washing on the basis of the blank group.The VAS pain score,AOFAS score of the ankle and hindfoot function,swing distance of pressure center and maximum dorsiflexion angle of ankle joint support period were compared between the two groups before and 3 months after operation.Results The VAS pain score,AOFAS score of the ankle and hindfoot function,swing distance of pressure center and maximum dorsiflexion angle of ankle joint support period in the two groups were better than those before operation(P<0.05).The VAS pain score,AOFAS score of the ankle and hindfoot function and maximum dorsiflexion angle of ankle joint in the control group were better than those in the control group at 3 months after operation(P<0.05).There was no significant difference in the swing distance of pressure center between the control group and the blank group at 3 months after operation(P>0.05).Conclusion Haitongpi Decoction has a significant effect on postoperative pain relief and ankle function improvement in patients with allogeneic tendon anatomical reconstruction of lateral ligament,and accelerates the recovery of patients.
