通心痹膏穴位敷贴联合耳穴压豆治疗稳定型心绞痛的临床效果

摘　　要：目的:探究通心痹膏穴位敷贴联合耳穴压豆治疗冠心病稳定型心绞痛(热毒痰瘀、气阴两虚证)的临床疗效、安全性。方法:选取2020年6月至2021年6月就诊于扬州大学临床中医学院(扬州市中医院)中医辨证属热毒痰瘀、气阴两虚型慢性稳定型心绞痛(CSAP)住院患者120例作为研究对象,采用SPSS 21.0统计软件随机工具进行简单随机化分为观察组和对照组,每组60例。对照组给予西药常规治疗,观察组在基础治疗的基础上加用通心痹膏穴位敷贴联合耳穴压豆治疗。分析2组患者治疗前后心绞痛发作频率及持续时间、中医证候积分、血清超敏C反应蛋白水平、血脂四项水平等相关观察指标,进行临床综合有效率评价;分析患者一般性体征、肝肾功能、血尿便、不良反应等指标,进行安全性评价。结果:2组治疗方案对CSAP患者均有效果,且观察组的临床综合效果优于对照组(P<0.05),观察组患者治疗后心绞痛发作频率及持续时间、中医证候积分、血清超敏C反应蛋白及血清总胆固醇、三酰甘油、低密度脂蛋白下降水平均优于对照组(均P<0.05),观察组血清高密度脂蛋白上升水平与对照组差异无统计学意义(P>0.05)。结论:通心痹膏穴位敷贴联合耳穴压豆可以改善热毒痰瘀、气阴两虚证CSAP患者临床症状,提高患者生命质量,对降低患者血清超敏C反应蛋白、血清总胆固醇、三酰甘油、低密度脂蛋白水平具有一定作用,且药物经济性较好,值得临床进一步推广。Objective:To explore the clinical efficacy and safety of topical application of TongXin Bi Ointment at acupoints combined with auricular acupressure bean-pressing in the treatment of stable angina pectoris of coronary heart disease(heat toxin phlegm stasis,qi and yin deficiency).Methods:A total of 120 hospitalized chronic stable angina pectoris(CSAP) patients with heat-toxin phlegm stasis,deficiency of both qi and yin from June 2020 to June 2021 at the Clinical School of Traditional Chinese Medicine,Yangzhou University(Yangzhou Traditional Chinese Medicine Hospital) were selected as the study subjects and randomly divided into the treatment group and the control group using SPSS 21.0 software,with 60 cases in each group.The control group was given conventional western medicine treatment,and the observation group received TongXinBi Ointment at acupoints combined with auricular acupressure bean-pressing on the basis of basic treatment.The frequency and duration of angina pectoris attack,TCM symptom score,serum high-sensitivity C-reactive protein level,and blood lipid level were analyzed as relevant observation indicators before and after treatment in the two groups to evaluate the clinical comprehensive efficacy;the general physical signs,liver and kidney function,hematuria and stool,adverse reactions and other indicators were analyzed for safety.Results:Both treatment regimens were effective for CSAP patients,and the clinical comprehensive efficacy of the observation group was superior to that of the control group(P<0.05).The frequency and duration of angina pectoris attack,TCM symptom score,serum high-sensitivity C-reactive protein and serum total cholesterol,triglyceride,low-density lipoprotein decreased levels of the observation group were better than those of the control group(P<0.05),and the serum high-density lipoprotein increased levels of the observation group were not different from those of the control group(P>0.05).Conclusion:The combination of topical application of TongXinBi Ointment at acupoints comb
