老年慢性阻塞性肺疾病急性加重再入院相关危险因素及焦虑抑郁评分分析

摘　　要：目的分析老年慢性阻塞性肺疾病急性加重(AECOPD)再入院相关危险因素及焦虑抑郁评分。方法选取我院2021年7月至2022年7月收治的280例老年AECOPD患者为研究对象。收集患者一般资料,并检测肺功能、进行焦虑抑郁评估。结果因AECOPD再入院者共92例,占比32.86%;未因AECOPD再入院者共188例,占比67.14%。因AECOPD再入院者出院后3个月内急性加重次数更多,FEV_(1)更低,HAMA评分、HAMD评分更高(P<0.05)。老年AECOPD再入院危险因素包括出院后3个月内急性加重次数、FEV_(1)及HAMA评分、HAMD评分(P<0.05)。结论老年AECOPD再入院与其出院后急性加重次数与肺功能密切相关,且患者普遍存在明显的焦虑抑郁情绪,临床上应对此加以重视并进行积极应对。Objective To analyze risk factors and anxiety and depression scores associated with readmission in older adults with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 280 elderly AECOPD patients admitted to our hospital from July 2021to July 2022 were selected for the study.General data of the patients were collected,and pulmonary function was tested and anxiety and depression assessments were performed.Results Those readmitted for AECOPD totaled 92 cases(32.86%),and those not readmitted for AECOPD totaled 188 cases(67.14%).Those readmitted for AECOPD had more acute exacerbations,lower FEV_(1),and higher HAMA scores and HAMD scores within 3 months after discharge(P<0.05).Elderly AECOPD readmission risk factors included the number of acute exacerbations within 3 months after discharge,FEV_(1),and HAMA score,HAMD score(P<0.05).Conclusions The readmission and the number of acute exacerbations after discharge of AECOPD in the elderly are closely related to lung function,and the patients generally have obvious anxiety and depression,which should be paid attention to and actively dealt with in clinic.
