摘 要:目的 研究血管生成素样蛋白2(ANGPTL2)在2型糖尿病肾病(T2DN)的炎症性损伤和纤维化中的机制。方法 将45只大鼠随机选取15只为正常对照组,剩余30只随机分为T2DN组、二甲双胍组各15只,建立T2DN模型,进行肾组织病理组织学分析,测定血糖及生化指标,测定炎性损伤及肾纤维化指标,检测ANGPTL2、Nephrin、Podocin表达,并分析ANGPTL2与炎性损伤及肾纤维化指标的相关性。结果 T2DN组血糖、24 h尿蛋白定量、血肌酐(Scr)、甘油三酯(TG)、总胆固醇(TC)均高于对照组、二甲双胍组,胰岛素低于其他两组(P <0.05);T2DN组大鼠肾组织可见明显病理改变,二甲双胍组病理损伤逐渐好转;TUNEL染色显示,T2DN组细胞凋亡指数较对照组、二甲双胍组增加(P <0.05);T2DN组肾组织匀浆中白细胞介素-6(IL-6)、C反应蛋白(CRP)、血管内皮生长因子(VEGF)、转化生长因子β1(TGF-β1)表达量高于对照组、二甲双胍组,二甲双胍组IL-6、VEGF、TGF-β1表达量高于对照组(P <0.05);T2DN组ANGPTL2 mRNA均高于对照组、二甲双胍组(P <0.05),但Nephrin、Podocin mRNA低于对照组、二甲双胍组(P <0.05);T2DN大鼠ANGPTL2 mRNA与IL-6、CRP、VEGF、TGF-β1呈正相关(P <0.05)。结论 ANGPTL2与T2DN的炎性损伤及纤维化有密切关联,可能和其下调Nephrin、Podocin有关。Objective To investigate the mechanism of angiopoietin-like protein 2(ANGPTL2) in inflammatory injury and fibrosis of type 2diabetic nephropathy(T2DN).Methods Fifteen of the 45 rats were randomly selected as the normal control group,and the remaining 30 rats were randomly divided into T2DN group and metformin group.T2DN model was established,histopathological analysis of renal tissue was performed,blood glucose and biochemical indicators were measured,inflammatory injury and renal fibrosis indicators were measured,and the expressions of ANGPTL2,Nephrin and Podocin were detected.The correlation between ANGPTL2 and inflammatory injury and renal fibrosis indicators was analyzed.Results Compared with the control and metformin groups,the T2DN group had significantly higher levels of blood glucose,24-hour urinary protein,serum creatinine(Scr),triglyceride(TG),and total cholesterol(TC) and significantly lower levels of insulin(P<0.05).Obvious pathological changes were observed in the renal tissue of the T2DN group,and the pathological damage was gradually improved in the metformin group.TUNEL staining showed that the apoptosis index of the T2DN group was significantly higher than that of the control group and the metformin group(P<0.05).The expression levels of interleukin-6(IL-6),Creactive protein(CRP),vascular endothelial growth factor(VEGF) and transforming growth factor-β1(TGF-β1) in renal tissue homogenate in T2DN group were higher than those in control group and metformin group,and the expression levels of IL-6,VEGF and TGF-β1 in metformin group were higher than those in control group(P<0.05).The mRNA levels of ANGPTL2 in T2DN group were significantly higher than those in control group and metformin group(P<0.05),but the mRNA levels of Nephrin and Podocin in T2DN group were significantly lower than those in control group and metformin group(P<0.05).ANGPTL2 mRNA in T2DN rats was positively correlated with IL-6,CRP,VEGF and TGF-β1(P<0.05).Conclusions ANGPTL2 is closely related to the inflammatory injury and fibr
