膈下逐瘀汤治疗晚期血吸虫病肝纤维化临床观察

摘　　要：目的探讨膈下逐瘀汤治疗晚期血吸虫病肝纤维化的临床效果。方法将120例晚期血吸虫病肝纤维化患者采用随机数字表法分为对照组(60例)和观察组(60例),对照组口服吡喹酮片治疗,观察组在此基础上联合膈下逐瘀汤治疗,疗程为6个月。比较两组临床疗效和治疗前后的肝功能、肝纤维化标志物、血清白细胞介素-13(IL-13)水平,以及两组治疗期间的不良反应。结果观察组的总体有效率为93.33%(56/60),明显高于对照组的80.00%(48/60)(P<0.05)。两组治疗前丙氨酸氨基转移酶(ALT)、天冬氨酸转氨酶(AST)、总胆红素(TBIL)、Ⅲ型前胶原肽(PⅢP)、Ⅳ型胶原(C-Ⅳ)、透明质酸(HA)、层黏连蛋白(LN)和IL-13水平比较差异无统计学意义(P>0.05);治疗后上述各项指标均较治疗前显著降低(P<0.05),且观察组明显低于对照组(P<0.05)。结论晚期血吸虫病肝纤维化患者在常规治疗基础上联合膈下逐瘀汤治疗,能够明显提高临床效果,减少肝损伤,降低血清IL-13水平,改善肝纤维化程度。Objective To investigate the efficacy of Gexia Zhuyu decoction in the treatment of advanced schistosomiasis liver fibrosis and its effect on serum IL-13 level.Methods A total of 120 patients with advanced schistosomiasis liver fibrosis were randomly divided into control group of 60 cases and observation group of 60 cases.The control group was treated with praziquantel tablets,while the observation group was treated with Gexia Zhuyu decoction on this basis.The course of treatment was 6 months.The clinical efficacy after treatment,liver function,liver fibrosis markers,serum IL-13 levels before and after treatment,and adverse reactions during treatment were compared between the two groups.Results The overall effective rate of the observation group was 93.33%(56/60),which was significantly higher than that of the control group(80.00%,48/60)(P<0.05).There were no significant differences in the lev-els of alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL),typeⅢprocollagen peptide(PⅢP),type IV collagen(C-Ⅳ),hyaluronic acid(HA),laminin(LN)and IL-13 between the two groups before treatment(P>0.05).After treatment,the above indexes were significantly lower than those before treatment(P<0.05),and the observation group was significantly lower than the control group(P<0.05).Conclusion On the basis of conventional treatment combined with Gexia Zhuyu decoction for advanced schistosomiasis patients with liver fibrosis can significantly improve the clinical efficacy,reduce liver injury,reduce serum IL-13 level,and improve the degree of liver fibrosis.
