活血通痹方膏摩治疗膝骨关节炎临床观察

摘　　要：目的观察自拟活血通痹方膏摩治疗膝骨关节炎(KOA)的临床效果。方法选取2021年7月—2022年7月于北京市石景山区中医医院骨科门诊治疗的KOA患者74例,随机分为试验组和对照组,试验组采用自拟活血通痹方药膏,通过膏摩治疗患膝,对照组采用双氯芬酸二乙胺乳胶剂,通过膏摩治疗患膝,每次15 min,14次为1个疗程,记录治疗前后患者膝关节视觉模拟量表(VAS)评分、中医证候评分以及西安大略和麦克马斯特大学骨关节炎指数(WOMAC)评分,进行疗效评价和对比。结果两组患者治疗后VAS评分、中医证候评分及WOMAC评分较治疗前均有降低,差异有统计学意义(P<0.05);试验组与对照组VAS评分比较差异无统计学意义(P>0.05),试验组中医证候评分及WOMAC评分均优于对照组,差异有统计学意义(P<0.05)。结论活血通痹方膏摩治疗KOA可有效缓解患者疼痛,改善其膝关节功能,延缓病情进展。Objective To observe the clinical effect of self-made Huoxue Tongbi prescription ointment on knee osteoarthritis(KOA).Methods A total of 74 KOA patients treated in Shijingshan Hospital of Traditional Chinese Medicine from July 2021 to July 2022 were randomly divided into experimental group and control group.The experimental group received self-prepared Huoxue Tongbi pre-scription ointment to treat the affected knee by ointment,and the control group received diclofenac diethyldiamine emulsion to treat the affected knee by ointment.The patients’knee joint visual analogue Scale(VAS)scores,TCM syndrome scores and Western On-tario and McMaster University Osteoarthritis Index(WOMAC)scores were recorded before and after treatment,15 min each time,14 times for 1 course of treatment,and the efficacy was evaluated and compared.Results VAS score,TCM syndrome score and WOMAC score of two groups were lower after treatment than before treatment,and the difference was statistically significant(P<0.05).There was no statistical significance in VAS scores between the experimental group and the control group(P>0.05),but TCM syndrome scores and WOMAC scores in the experimental group were better than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Poultice massage by Huoxue Tongbi prescription ointment has satisfactory effect on KOA.Compared with diclofenac diethylamine emulgel,it can more efficiently improve knee function and release stiffness of knee in short-medium term.It is worthy of promotion and application.
