达格列净对早期2型糖尿病肾病患者肾功能的影响

摘　　要：目的探讨达格列净对早期2型糖尿病肾病(DN)患者肾功能的影响。方法选取2020年1月份至2021年1月山东中医药大学二附院收诊的102例早期2型DN患者作为研究对象,按照收诊先后次序分为应用组和对比组,每组51例。对比组在辅助治疗基础上口服二甲双胍缓释片,应用组在辅助治疗基础上口服达格列净片,比较两组治疗前后生化指标[总胆固醇(TC)、甘油三酯(TG),血肌酐(Scr)、尿素氮(BUN)、糖化血红蛋白(HbA1c)、空腹血糖(FBG),餐后2小时血糖(2 h PBG)]、血清胱抑素-C(Cys-C)及尿液指标[尿微量白蛋白(24 h UAE)、N-乙酰-β-氨基葡萄糖苷酶(NAG),β2-微球蛋白(β2-MG)]。结果治疗后,两组TC、TG、HbA1c、FBG及2 h PBG水平均低于治疗前,且应用组TC、2 h PBG水平均低于对比组,差异有统计学意义(P<0.05),其余两组间及组内比较差异无统计学意义。治疗后,两组Cys-C、NAG、β2-MG、24 h UAE水平均低于治疗前,且应用组低于对比组,差异有统计学意义(P<0.05)。结论达格列净治疗DN患者能延缓病情、控制血糖,减轻患者肾小管损伤,保护肾功能,值得临床推广应用。Objective To investigate the effect of dapagliflozin on renal function in patients with early type 2 diabetic nephropathy(DN).Methods 102 patients with early type 2 DN admitted to the Second Affiliated Hospital of Shandong University of Traditional Chinese Medicine from January 2020 to January 2021 were selected as the research subjects,they were divided into the application group and the control group accord-ing to the order of admission,with 51 cases in each group.The control group was treated with metformin sustained-release tablets on the basis of ad-juvant therapy,and the application group was treated with dapagliflozin tablets on the basis of adjuvant therapy.The biochemical indexes(total cho-lesterol[TC],triglyceride[TG],serum creatinine[Scr],urea nitrogen[BUN],glycosylated hemoglobin[HbA1c],fasting blood-glucose[FBG],2 h postprandial blood glucose[2 h PBG]),cystatin-C(Cys-C)and urine indexes(urinary microalbumin[24 h UAE],N-acetyl-β-glucosaminidase[NAG],β2-microglobulin[β2-MG])were compared between the two groups before and after treatment.Results After treatment,the levels of TC,TG,HbA1c,FBG and 2 h PBG in the two groups were lower than those before treatment,and the levels of TC and 2 h PBG in the application group were lower than those in the control group,the differences were statistically significant(P<0.05),there was no significant difference be-tween the other two groups and within the group.After treatment,the levels of Cys-C,NAG,β2-MG and 24 h UAE of the two groups were lower than those before treatment,and those in the application group were lower than the control group,the differences were statistically significant(P<0.05).Conclusion Dapagliflozin can delay the condition,control blood glucose,reduce renal tubular injury and protect renal function in patients with DN,which is worthy of clinical promotion and application.
