摘  要:目的:应用细菌鉴定药敏分析系统明确某医院院内获得性肺炎下呼吸道感染患者的病原菌分布,验证病原菌的耐药性,在现有常规抗菌药物情况下,为临床前期治疗合理使用抗菌药物提供参考。方法:将某医院2020年1月~2022年12月院内患者中肺炎合并下呼吸道感染患者135例作为研究对象,首先收集患有此病患者的痰液标本,之后再对标本进行普通培养及药敏试验分析。流程上需要对采集的患者痰液样本进行病原菌的分离,并且及时地观测病原菌的分布状况,之后则对从患者痰液内检测到的病原菌进行耐药性测试。结果:在收集到的135例患者的135份痰样本中共分离检测出了288株病原菌,有革兰阳性球菌、革兰阴性杆菌以及真菌,其中数目最多的病原菌就是革兰阴性杆菌,共鉴定出231株,占80.21%,革兰阳性球菌位列第二,共有31株病原菌,占10.76%,真菌占比最少。在革兰阴性杆菌类群中,分布最为广泛的是肺炎克雷伯菌,共分离鉴定出98株,占42.42%;而在革兰阳性球菌中,所占比例最多的病原菌是金黄色葡萄球菌,共鉴定出17株,占54.84%;在本试验中进行的病原菌耐药性试验中,革兰阴性杆菌中的铜绿假单胞菌对本次试验中选取的抗菌药物的耐药性比较强,对临床常用药复方新诺明的耐药率为80.85%;革兰阳性球菌中数量最多的金黄色葡萄球菌对青霉素的耐药性表现得也比较高,为82.35%。结论:应用细菌鉴定药敏分析系统可发现在本院院内获得性肺炎下呼吸道感染常见的病原菌主要是革兰阴性杆菌,细菌对抗菌药物的耐药性比较高,在临床上对患者进行治疗时,需要考虑到病原菌的耐药性,合理选择现有常规抗菌药物,提高患者的治疗疗效。Objective:To clarify the distribution of pathogenic bacteria in patients with lower respiratory tract infection of nosocomial acquired pneumonia in a hospital,to verify the drug resistance of pathogenic bacteria.Methods:A total of 135 patients with pneumonia and lower respiratory tract infection in a certain hospital from January 2020 to December 2022 were selected as the research subjects.First,sputum samples of patients with this disease were collected,and then the samples were subjected to general culture and drug sensitivity analysis.In the process,it is necessary to isolate pathogenic bacteria from the collected patient sputum samples and observe the distribution of pathogenic bacteria in a timely manner.Then,drug resistance testing should be conducted on the pathogenic bacteria detected from the patient sputum.Results:A total of 288 pathogenic bacteria were isolated and detected in 135 sputum samples collected from 135 patients,including Gram positive cocci,Gram negative bacilli,and fungi.Among them,the most numerous pathogenic bacteria were Gram negative bacilli,with a total of 231 strains identified,accounting for 80.21%.Gram positive cocci ranked second,with a total of 31 pathogenic bacteria,accounting for 10.76%,and fungi accounting for the least.Among the Gram negative bacterial groups,the most widely distributed is Klebsiella pneumoniae,co isolated 98 strains were identified,accounting for 42.42%;Among Gram positive cocci,the pathogen with the highest proportion is Staphylococcus aureus,with a total of 17 strains identified,accounting for 54.84%;In the pathogen resistance test conducted in this experiment,Pseudomonas aeruginosa in Gram negative bacteria showed strong resistance to the selected antibiotics in this experiment,with a resistance rate of 80.85%to the commonly used clinical drug compound Xinnuomin;Staphylococcus aureus,which has the highest number of Gram positive cocci,also shows relatively high resistance to penicillin,accounting for 82.35%.Conclusion:The application of the bacterial iden
