呼吸神经肌肉刺激仪联合正压机械通气应用于呼吸衰竭患者的疗效及对其动脉血气指标水平的影响状况分析

摘　　要：目的:探讨呼吸衰竭予以呼吸神经肌肉刺激仪+正压机械通气疗法对其动脉血气的改善价值。方法:研究样本源于2022年10月~2023年6月邳州市第十人民医院收治的呼吸衰竭患者80例,以随机数字表分为对照组与观察组,各40例,前者实施正压机械通气、后者予以(呼吸神经肌肉刺激仪+正压机械通气),观察患者动脉血气、呼吸力学参数、呼吸困难度及疲劳量表评分等变化。结果:治疗后,较之对照组,观察组动脉血二氧化碳分压水平更低,动脉血氧分压及动脉血氧饱和度水平更高(P<0.05);观察组平均气道压与平台压水平较对照组低,吸气末跨肺压与静态顺应性水平较对照组高(P<0.05);观察组呼吸困难量表与疲劳量表评分均低于对照组(P<0.05)。结论:对呼吸衰竭患者选用呼吸神经肌肉刺激仪+正压机械通气治疗,可有效改善患者动脉血气指标水平,获得显著疗效。Objective:To explore the value of improving arterial blood gas(ABG)in patients with respiratory failure treated with respiratory nerve muscle stimulator and positive pressure mechanical ventilation.Methods:The study sample was 80 patients with respiratory failure admitted to the Tenth People's Hospital of Pizhou City from October 2022 to June 2023.They were randomly divided into a control group and an observation group,with 40 cases in each group,using a random number table.The former implements positive pressure mechanical ventilation,while the latter receives respiratory neuromuscular stimulator+positive pressure mechanical ventilation.The changes in arterial blood gas,respiratory mechanics parameters,respiratory distress,and fatigue scale scores of the patient are observed.Results:After treatment,compared with the control group,the observation group had lower levels of arterial carbon dioxide partial pressure,higher levels of arterial oxygen partial pressure and arterial oxygen saturation(P<0.05);The average airway pressure and plateau pressure levels in the observation group were lower than those in the control group,while the final inspiratory transpulmonary pressure and static compliance levels were higher than those in the control group(P<0.05);The respiratory distress scale and fatigue scale scores of the observation group were lower than those of the control group(P<0.05).Conclusion:The patients with respiratory failure were treated with respiratory nerve muscle stimulator and positive pressure mechanical ventilation,which could effectively improve the arterial blood gas index of the patients.
