多层螺旋CT灌注成像与增强磁共振成像在早期肝癌诊断中应用效能对比

摘　　要：目的:探讨多层螺旋CT(MSCT)灌注成像与增强磁共振成像(DCE-MRI)在早期肝癌诊断中的应用价值。方法:选取2021年1月~2023年10月本院收治的86例疑似肝癌患者,均行MSCT灌注成像、DCE-MRI检查,并以病理检查为金标准,分析肝癌检出率及诊断效能,并以Kappa检验验证不同方法诊断与金标准的一致性。结果:86例疑似患者金标准确诊45例,其中MSCT灌注成像检出44例,阳性率为97.78%(44/45);DCE-MRI检出40例,阳性率为88.89%(40/45);MSCT灌注成像诊断灵敏度、准确度高于DCE-MRI(P<0.05);Kappa检验显示,MSCT灌注成像诊断与金标准的Kappa值为0.930,一致性极高;DCE-MRI诊断与金标准的Kappa值为0.745,一致性尚可。结论:MSCT灌注成像在早期肝癌诊断中价值更高,可降低漏诊、误诊风险,实现早期精准治疗。Objective:To explore the application value of multislice spiral CT(MSCT)perfusion imaging and enhanced magnetic resonance imaging(DCE-MRI)in the diagnosis of early liver cancer.Methods:86 patients with suspected hepatocellular carcinoma admitted to our hospital from January 2021 to October 2023 were selected,and all of them underwent MSCT perfusion imaging and DCE-MRI with pathological examination as the gold standard to analyze the detection rate of hepatocellular carcinoma and diagnostic efficacy,and the consistency of diagnosis of different methods with the gold standard was verified by Kappa test.Results:The gold standard of 86 suspected patients confirmed the diagnosis of 45 cases,of which 44 cases were detected by MSCT perfusion imaging,with a positive rate of 97.78%(44/45);40 cases were detected by DCE-MRI,with a positive rate of 88.89%(40/45);The diagnostic sensitivity and accuracy of MSCT perfusion imaging were higher than that of DCE-MRI(P<0.05);The Kappa test showed that the diagnosis of MSCT perfusion imaging was highly consistent with the gold standard Kappa value of 0.930;The Kappa value between DCE-MRI diagnosis and the gold standard is 0.745,and the consistency is still acceptable.Conclusion:MSCT perfusion imaging is of higher value in the diagnosis of early hepatocellular carcinoma,which can reduce the risk of missed diagnosis and misdiagnosis,and realize early precision treatment.
