基于血瘀理论探析子宫内膜异位症的病理机制与物质基础研究

摘　　要：子宫内膜异位症具有形态多样性、病变广泛性和恶性生物学行为等致病特点,给临床治疗带来诸多困难。通过对古今文献资料的复习与梳理,基于血瘀相关理论从病名源流、发病机制、证候要素和生物学基础等方面深入探讨子宫内膜异位症的病理机制,发现“血瘕”“癥瘕”与子宫内膜异位症高度契合,认为“瘀血阻滞冲任、胞宫”是子宫内膜异位症关键病机,气滞、寒凝、痰湿、气虚和肾虚等是瘀血阻滞的诱因或后续病机,血瘀证为该病的基础证候,其中细胞增殖或血管形成因子、肿瘤标志物、血管收缩因子和某些差异蛋白等与该病血瘀证具有特异相关性,这些可能为探寻子宫内膜异位症血瘀证的物质基础提供了依据。Endometriosis is a disease characterized by morphological diversity,extensive lesions,malignant biological behavior,etc.,which bring many difficulties to clinical treatment.By reviewing and sorting out related ancient and modern literature and based on the theory of blood stasis,the pathological mechanism of endometriosis was discussed in depth from the origin of disease name,pathogenesis,syndrome factors and biological basis.Results showed that"abdominal masses due to blood stasis"or"aggregation accumulation"is highly consistent with endometriosis.It is believed that"blood stasis obstructing the thoroughfare and conception vessel and the uterus"is the key pathogenesis of endometriosis.Qi stagnation,cold congealing,phlegm-dampness,Qi deficiency and kidney deficiency are precipitating factors or subsequent pathogenesis of blood stasis.Blood stasis is the basic syndrome of the disease.Cell proliferation or angiogenesis factors,tumor markers,vasoconstriction factors and some differential proteins are specifically related to the blood stasis syndrome of the disease.These may provide a basis for exploring the material basis of blood stasis syndrome in endometriosis.
