超声引导下针刀神经触激术治疗神经根型颈椎病的临床观察

摘　　要：目的:观察在超声引导下针刀神经触激术治疗神经根型颈椎病的临床疗效。方法:按照随机数字表法将纳入的60例神经根型颈椎病患者分为观察组和对照组,各30例。观察组采取针刀神经触激术治疗,每周1次,治疗3周;对照组采取针刺治疗,每日1次,治疗3周。分别于治疗前、治疗1周和3周后观察2组患者视觉模拟评分(VAS)、颈椎功能障碍指数(NDI)评分、田中靖久评分、尺神经与正中神经的F波出现率及传导速度变化。结果:与对照组比较,观察组在治疗1周后、3周后,VAS评分、NDI评分、颈部田中靖久评分、尺神经与正中神经F波出现率、尺神经与正中神经神经传导速度均明显优于对照组(P<0.05)。观察组总有效率为93.33%,对照组总有效率为86.66%,观察组总有效率优于对照组(P<0.05)。结论:超声引导下针刀神经触激术治疗神经根型颈椎病安全有效,优于针刺治疗。Objective:To observe clinical effectiveness of ultrasound-guided acupotomy nerve stimulation in the treatment of cervical spondylotic radiculopathy.Methods:According to random number table method,60 patients with cervical spondylotic radiculopathy were divided into observation group and control group,with 30 cases in each group.The observation group was treated with acupotomy nerve stimulation once a week for 3 weeks,while the control group was treated with acupuncture once a day for 3 weeks.Visual analog scale(VAS),neck disability index(NDI),Tanaka Yasuhisa score,F-wave occurrence rate and conduction velocity of ulnar nerve and median nerve were observed before treatment,1 week and 3 weeks after treatment.Results:Compared with the control group,after 1 week and 3 weeks of treatment,the VAS score,NDI,Yasuhisa Tanaka score,F-wave occurrence rate and conduction velocity of ulnar nerve and median nerve of observation group was significantly better than that of the control group(P<0.05).The total effective rate of the observation group was 93.33%;the total effective rate of the control group was 86.66%,and the total effective rate of the observation group was better than that of the control group(P<0.05).Conclusion:Ultrasound-guided acupotomy nerve stimulation in the treatment of cervical spondylotic radiculopathy is safe and effective,which is better than acupuncture.
