母血DCN、sEng、PLGF水平与早发型胎儿生长受限的相关性分析

摘  要:目的:分析母血核心蛋白聚糖(DCN)、可溶性内皮因子(sEng)、胎盘生长因子(PLGF)与早发型胎儿生长受限(FGR)的相关性。方法:选取2021年1—12月于湖南省妇幼保健院分娩的120例FGR孕妇作为研究对象,其中早发型FGR 42例纳入早发型FGR组,晚发型FGR 78例纳入晚发型FGR组。选取同期分娩正常胎儿的120例孕妇作为对照组。比较三组母血血清DCN、sEng、PLGF水平及三组新生儿生长发育指标,分析母血血清DCN、sEng、PLGF水平与早发型新生儿生长发育指标的相关性。结果:早发型FGR组DCN、sEng水平高于晚发型FGR组、对照组,PLGF水平低于晚发型FGR组、对照组,差异有统计学意义(P<0.05);晚发型FGR组、对照组DCN、sEng、PLGF水平比较,差异无统计学意义(P>0.05)。三组新生儿体质量、腹围、头围、股骨长比较,早发型FGR组<晚发型FGR组<对照组,差异有统计学意义(P<0.05)。DCN、sEng水平与早发型FGR新生儿体质量、腹围、头围、股骨长呈负相关(P<0.05);PLGF与早发型FGR新生儿体质量、腹围、头围、股骨长呈正相关(P<0.05)。结论:母血DCN、sEng、PLGF水平与早发型FGR存在相关性,可用于早发型FGR的筛查、诊断。Objective:To analyze the correlation between maternal blood core proteoglycan(DCN),soluble endothelial factor(sEng),placental growth factor(PLGF)and early-onset fetal growth restriction(FGR).Methods:A total of one hundred and twenty pregnant women with FGR who gave birth in Hunan Maternal and Child Health Hospital from January 2021 to December 2021 were selected as the study objects.Among them,forty-two cases with early-onset FGR were included in the early-onset FGR group and seventy-eight cases with late-onset FGR were included in the late-onset FGR group.One hundred and twenty pregnant women with normal fetuses were selected as the control group.The levels of serum DCN,sEng and PLGF in the three groups and the growth and development indexes of the three groups were compared.The correlation between the levels of DCN,sEng and PLGF in maternal blood and the growth and development indexes of early-onset newborns were analyzed.Results:DCN and sEng levels in early-onset FGR group were higher than those in late-onset FGR group and control group,and PLGF levels were lower than those in late-onset FGR group and control group,the difference was statistically significant(P<0.05);there was no significant difference in DCN,sEng and PLGF levels between the late-onset FGR group and the control group(P>0.05);the body weight,abdominal circumference,head circumference and femur length of the three groups were compared,the early-onset FGR group was<the late-onset FGR group was<the control group,the difference was statistically significant(P<0.05);DCN and sEng levels were negatively correlated with body mass,abdominal circumference,head circumference and femur length of newborns with early-onset FGR(P<0.05);PLGF was positively correlated with body weight,abdominal circumference,head circumference and femur length of newborn with early-onset FGR(P<0.05).Conclusion:The levels of maternal blood DCN,sEng and PLGF are correlated with early-onset FGR,which can be used for screening and diagnosis of early-onset FGR.
