摘 要:目的:分析15例样本嗜酸性粒细胞假性增高原因并提出解决方法。方法:采用XN1000全自动血细胞分析仪进行测定,对于仪器报警散点图异常样本延长放置时间复查,复测仍报警样本进行血涂片瑞姬染色人工镜检分类。结果:15例报警散点图异常嗜酸性粒细胞增高样本中9例延长放置30 min后复测仪器报警消除,6例延长放置30 min后复测仪器仍然报警。延长放置30 min后复测仪器报警消除样本各类白细胞群散点图分布正常,界限清晰,无融合。嗜酸性粒细胞比例降至正常范围,中性粒细胞比例升高。放置30 min后复测仪器仍然报警样本白细胞分类散点图,红色的嗜酸性粒细胞群与蓝色的中性粒细胞群区域之间出现粘连融合,呈现灰色散点分布。结论:全自动血液分析仪测定原理保证了仪器检测结果的敏感性和稳定性,但仪器出现特殊报警,仍然需要人工镜检,同时出现异常白细胞散点图时,在排除疾病影响的情况下,还应考虑仪器的原因。Objective:To analyze the cause of false increase of eosinophils in 15 samples and propose solutions.Methods:XN1000 automatic hematology analyzer was used for assay.The samples triggering an instrument alarm with abnormalities in scatter plots were re-tested after an extended holding time,and the re-tested samples that still triggered an alarm were subject to blood smear for Wright-Giemsa staining and were classified by manual microscopic examination.Results:Among the 15 samples with eosinophil increase triggering an alarm with abnormalities in scatter plots,9 samples were re-tested after the holding time was extended for 30 min and the instrument alarm was eliminated,and 6 samples were re-tested after the holding time was extended for 30 min and the instrument alarm was still present.After the holding time was extended for 30 min,for the samples with the instrument alarm eliminated by retest,various white blood cell populations showed normal distribution in the scatter plots,with clear boundaries and with no fusion.The proportion of eosinophils decreased to the normal range,and the proportion of neutrophils increased.After the holding time was extended for 30 min,for the samples with the instrument alarm present by retest,white blood cell differential scatter plots showed the presence of adhesions and fusions between the red eosinophil population and the blue neutrophil population,in a gray scatter distribution.Conclusion:The principle of automatic hematology analyzer assay ensures the sensitivity and stability of the instrument testing results.Once the special alarm of the instrument occurs,manual microscopic examination is still required.When the abnormal white blood cell scatter plot appears,the instrument factor should also be considered in the case of excluding the influence of the disease.
