摘 要:目的探究血离子钙(iCa)对新生儿脓毒症病情的评估价值。方法回顾性分析2019年7月至2022年3月赣南医科大学第一附属医院收治的83例脓毒症患儿的病历资料,根据病情严重程度分为脓毒症组(53例)和严重脓毒症组(30例);比较脓毒症组与严重脓毒症组患儿iCa、C-反应蛋白(CRP)、清蛋白(ALB)水平;采用Pearson相关分析iCa水平与ALB、CRP水平和NCIS的相关性。结果脓毒症组与严重脓毒症组患儿iCa、ALB和CRP水平比较,差异均有统计学意义(Z=-5.169,P<0.001;t=2.225,P=0.029;t′=-11.655,P<0.001)。Pearson相关分析结果显示,iCa水平与ALB水平和NCIS呈正相关(r=0.445,P<0.001;r=0.480,P<0.001),与CRP水平呈负相关(r=-0.537,P<0.001)。依据新生儿低钙血症定义对iCA的界定,将83例脓毒症患儿分为iCa≥1.0 mmol/L组(57例)与iCa<1.0 mmol/L组(26例),iCa≥1.0 mmol/L组和iCa<1.0 mmol/L组患儿ALB、CRP水平及病情危重程度比较,差异均有统计学意义(t=3.859,P<0.001;Z=-4.605,P<0.001;Z=-3.741,P<0.001)。结论iCa水平与脓毒症新生儿的病情严重程度相关,与CRP水平呈负相关,与ALB水平、NCIS呈正相关,病情越重,iCa水平越低。Objective To explore the value of serum ionized calcium(iCa)in the evaluation of neonatal sepsis.Methods The clinical data of 83 neonates with sepsis who met the inclusion criteria and were admitted to the First Affiliated Hospital of Gannan Medical University from July 2019 to March 2022 were retrospectively analyzed.According to the severity of the disease,the patients were divided into sepsis group(53 cases)and severe sepsis group(30 cases).The differences of iCa,C-reactive protein(CRP)and albumin(ALB)levels were compared between the sepsis group and the severe sepsis group.Pearson correlation was used to analyze the correlation of iCa levels with ALB,CRP levels and NCIS score.Results Comparison of iCa,ALB and CRP levels between the sepsis group and severe sepsis group showed statistically significant differences(Z=-5.169,P<0.001;t=2.225,P=0.029;t′=-11.655,P<0.001).Pearson correlation analysis results showed that iCa level was positively correlated with ALB level and NCIS score(r=0.445,P<0.001;r=0.480,P<0.001)and negatively correlated with CRP level(r=-0.537,P<0.001).According to the definition of iCa based on the definition of neonatal hypocalcemia,83 neonates with sepsis were divided into 57 cases in the iCa≥1.0 mmol/L group and 26 cases in the iCa<1.0 mmol/L group,and the differences of ALB,CRP levels and critical grades of the disease between the two groups were statistically significant(t=3.859,P<0.001;Z=-4.605,P<0.001;Z=-3.741,P<0.001).Conclusion iCa level is correlated with the severity of neonatal sepsis,negatively correlated with critical grades of the disease and CRP level,and positively correlated with ALB.The more severe the condition,the lower the level of iCa.
