情绪状态对腰椎间盘突出症患者成人依恋、疼痛调适的影响

摘　　要：目的探讨情绪状态在腰椎间盘突出症患者成人依恋和疼痛调适间的中介效应。方法采用一般资料调查表、正负情绪量表、整体疼痛评估量表、亲密关系经历量表简版,对2021年5月至2022年5月收治腰椎间盘突出症患者进行调查。通过多元线性回归和结构方程模型评价情绪状态在成人依恋和疼痛调适间的中介效应。结果本研究最终纳入95例腰椎间盘突出症患者,其亲密关系经历量表简版中依恋焦虑和依恋回避得分分别为(47.56±10.15)分和(63.68±10.37)分,情绪状态总分为(54.75±3.80)分,疼痛调适总分(76.17±21.40)分。依恋焦虑和依恋回避与负性情绪以及疼痛调适得分呈正相关(P<0.05),与正性情绪得分、情绪状态总分呈负相关(P<0.05)。依恋焦虑和依恋回避通过情绪状态对腰椎间盘突出症患者疼痛调适的间接效应显著,情绪状态的中介效应分别占总效应的18.9%和13.6%。结论情绪状态在腰椎间盘突出症患者成人依恋和疼痛调适间起中介效应,护理人员应以情绪状态为干预切入点,以减少成人依恋对疼痛调适带来的不良影响。Objective To explore the mediating effect of emotional state between adult attachment and pain adjustment in patients with lumbar disc herniation(LDH).Methods LDH patients admitted from May 2021 to May 2022 were surveyed using the general information questionnaire,Positive and Negative Affect Scale(PANAS),Global Pain Scale(GPS),and Experiences in Close Relationships Scale-Short Form(ECR-S).Multivariate linear regression and structural equation model were implemented to assess the mediating effect of emotional state on adult attachment and pain adjustment in LDH patients.Results Totally 95 patients were enrolled in the present study.The ECR-S scores of attachment avoidance and attachment anxiety were(47.56±10.15)points and(63.68±10.37)points,respectively.The scores of PANAS and GPS were(54.75±3.80)points and(76.17±21.4)points,respectively.Attachment anxiety and attachment avoidance were positively correlated with negative emotion and pain adjustment scores(P<0.05),and negatively correlated with positive emotion and total emotional state scores(P<0.05).Attachment anxiety and attachment avoidance had a significantly indirect effect on the pain adjustment of LDH patients through emotional state,showing a 18.9% and 13.6% mediating effect of emotional state on the total effect,respectively.Conclusion Emotional state play a mediating role in the correlation between adult attachment and pain adjustment in LDH patients.Nursing care providers can focus on emotional state to reduce the adverse effects of adult attachment on pain adjustment.
