摘　　要：目的:分析心内血性囊肿的临床与病理特征,以提高对罕见心脏肿瘤的认识。方法:收集首都医科大学附属北京安贞医院2015年3月至2023年5月,诊断的6例心内血性囊肿病例,进行临床资料分析与病理形态学特征观察。结果:女性2例,男性4例;年龄25~59岁,中位年龄52.5岁。血性囊肿位于二尖瓣3例、主动脉瓣1例、三尖瓣1例、右心房1例。组织病理学表现为囊壁由纤维结缔组织构成,衬覆血管内皮细胞,腔内可见出血及血栓形成。免疫组织化学染色示CD31(+)、CD34(+)、D2-40(-)、Calretinin(-)。所有病例均完整切除,术后随访6例,均健在、无复发。结论:心内血性囊肿是极其少见的心脏良性肿瘤,术前通过超声心动图检查可高度提示此肿物可能,确诊需组织病理学检查。本病手术完整切除后通常预后良好。Objective:Analysis of clinical and pathological features of intracardiac blood cysts to improve the understanding of rare cardiac tumors.Methods:Six cases of intracardiac blood cysts diagnosed between March 2015 and May 2023 were collected for clinical data analysis and observation of pathomorphologic features,and combined with literature review.Results:A total of two females and four males were included,with an age range of 25 to 59 years and a median age of 52.5 years.The blood cysts were located in the mitral valve in 3 cases,the aortic valve in 1 case,the tricuspid valve in 1 case,and the right atrium in 1 case.Histopathologically,the wall of the capsule is composed of fibrous connective tissue lined with vascular endothelial cells,and hemorrhage and thrombosis are seen in the lumen.Immunohistochemical staining showed CD31(+),CD34(+),D2-40(-),Calretinin(-).All cases were completely resected,and six were followed up after surgery,all of which were alive and recurrence-free.Conclusions:Intracardiac blood cysts are extremely rare benign cardiac tumors,and preoperative examination by echocardiography is highly suggestive of the possibility of this mass,with histopathological examination required for definitive diagnosis.The prognosis for this disease is usually good after complete surgical resection.
