中药热奄包联合中药足浴对肿瘤患者顺铂化疗延迟性呕吐的影响

摘　　要：目的:探究中药热奄包联合中药足浴对肿瘤患者顺铂化疗延迟性呕吐的影响。方法:将120例肿瘤化疗延迟性呕吐患者按随机数字表法分为观察组和对照组各60例,两组均严格按照医嘱服用止吐药(地塞米松+肌注苯海拉明),对照组于化疗当日开始予中药足浴,观察组在对照组基础上于化疗当日开始加用中药热奄包干预,均连续干预5天。干预结束后比较两组患者呕吐程度、呕吐缓解率及干预前后负性情绪。结果:干预后,观察组患者呕吐缓解率为90.91%(50/55),高于对照组的76.79%(43/56)(P<0.05);干预后观察组患者呕吐频次少于对照组(P<0.05)、呕吐时间短于对照组(P<0.05);干预后两组患者汉密尔顿焦虑量表、汉密尔顿抑郁量表评分均降低(P<0.05),观察组低于对照组(P<0.05)。结论:中药热奄包联合中药足浴护理可提高肿瘤患者顺铂化疗延迟性呕吐缓解率,减少呕吐频次,缩短呕吐持续时间,有助于缓解患者焦虑及抑郁情绪。Objective:To explore the influence of Chinese medicine fomentation combined with traditional Chinese medicine footbath on delayed vomiting in tumor patients treated with cisplatin chemotherapy.Methods:All 120 patients with delayed vomiting after chemotherapy were randomly divided into observation group and control group,both groups used antiemetic drugs strictly(dexamethasone,muscular injection of benadryl)according to the doctor's prescription,and the control group was given traditional Chinese medicine footbath on the day of chemotherapy for five days.The observation group adopted Chinese medicine fomentation on the day of chemotherapy on the foundation of the therapy the control group received.To compare the degree of vomiting,the rate of vomiting relief and negative emotions before and after the intervention between the two groups.Results:After the intervention,the vomiting relief rate of patients in the observation group was 90.91%(50/55),higher than 76.79%(43/56)of the control group(P<0.05);the frequency of vomiting of patients in the observation group was less than that of the control group after the intervention(P<0.05),and the duration of vomiting was shorter than that of the control group(P<0.05);the scores of the Hamilton rating scale for Anxiety(HAMA)and the Hamilton rating scale for depression(HAMD)were reduced after the intervention in the two groups(P<0.05),and the observation group was lower than the control group(P<0.05).Conclusion:Traditional Chinese medicine fomentation combined with traditional Chinese medicine footbath can improve the remission rate of delayed vomiting in tumor patients with cisplatin chemotherapy,reduce the frequency of vomiting and the duration of vomiting,and help alleviate anxiety and depression.
