摘  要:目的 探讨阿尔茨海默病风险人群的超早期脑脊液β淀粉样蛋白(Aβ)沉积与默认网络(DMN)功能连通性的关系。方法 纳入阿尔茨海默病神经影像学计划从2010年7月至2018年12月在北美地区招募的轻度认知障碍受试者,根据36个月随访期脑脊液Aβ的变化将受试者分为Aβ下降组(25例)和稳定组(19例)。运用脑功能MRI技术研究脑脊液Aβ下降、DMN功能连通性和认知功能变化的关系。结果 相较于Aβ稳定组,Aβ下降组在随访过程中连线测试-B(TMT-B)时间延长(P<0.05)。在双侧枕中回和左侧颞中回,Aβ下降组DMN功能连通性低于Aβ稳定组(P<0.01);其中,左侧颞中回的功能连通性与TMT-B时间呈负相关(r<0,P<0.05)。结论 超早期Aβ沉积与颞枕叶DMN功能连通性下降密切相关,且导致了执行功能损害。Objective To investigate the relationship between ultra-early cerebrospinal fluid amyloid beta(Aβ)deposition and functional connectivity of the default network(DMN)in people at risk of Alzheimer’s disease.Methods Participants with mild cognitive impairment enrolled in the Alzheimer’s Disease Neuroimaging Program in North America from July 2010 to December 2018 were divided into Aβdeclining group(25 cases)and Aβstable group(19 cases)based on changes in cerebrospinal fluid Aβduring 36 months of follow-up.Functional brain MRI was used to study the relationship between cerebrospinal fluid Aβdecline,DMN functional connectivity and cognitive function.Results Compared with Aβstable group,the time of trail making test-B(MTT-B)in Aβdeclining group was longer during follow-up(P<0.05).In bilateral middle occipital gyrus and left middle temporal gyrus,DMN functional connectivity in Aβdeclining group was significantly lower than that in Aβstable group(P<0.01).The functional connectivity of the left medial temporal gyrus was negatively correlated with MTT-B time(r<0,P<0.05).Conclusion Ultra-early Aβdeposition is closely related to the decrease of temporal occipital DMN functional connectivity,and leads to the impairment of executive function.
