摘　　要：目的:利用沿血管周围间隙弥散张量成像(DTI-ALPS)评估急性缺血性脑梗死(AICI)患者的脑胶质淋巴系统功能变化。方法:回顾性搜集徐州医科大学附属医院2021年9月至2023年6月收治的33例AICI患者(AICI组)和33例健康志愿者(健康对照组)的临床及影像学资料,采集所有被试者的DTI参数以计算DTI-ALPS指数,用于评价AICI患者脑胶质淋巴系统功能的变化。结果:AICI组患者病灶侧大脑半球DTI-ALPS指数显著低于健侧(t=-3.11,P<0.01),也显著低于健康对照组(t=-5.20,P<0.01),AICI病灶的体积、病灶至测量层面的距离与病灶侧大脑半球DTI-ALPS指数均无显著相关性(P=0.38;P=0.24)。结论:利用DTI-ALPS指数能够评估AICI患者脑胶质淋巴系统功能变化,且AICI患者脑胶质淋巴系统功能的损害在病灶同侧大脑半球。Purpose:To evaluate the functional changes of the glial lymphatic system in patients with acute ischemic cerebral infarction(AICI)by diffusion tensor image analysis along the perivascular space(DTI-ALPS).Methods:The clinical and imaging data of 33 AICI patients(AICI group)and 33 healthy volunteers(healthy controls,HCs)in the Affiliated Hospital of Xuzhou Medical University from September 2021 to June 2023 were retrospectively collected.DTI parameters of all subjects were collected to calculate the DTI-ALPS index,which was used to evaluate the changes in the function of the glial lymphatic system in patients with AICI.Results:The DTIALPS index of cerebral hemispheres on the side of AICI lesions was significantly lower than those on the unaffected side(t=-3.11,P<0.01),and lower than those of HCs(t=-5.20,P<0.01).There was no significant correlation between lesion volume,distance from lesion to measurement level and DTI-ALPS index of cerebral hemisphere on the side of AICI lesions(P=0.38;P=0.24).Conclusions:The DTI-ALPS index can be used to evaluate the functional changes of the glial lymphatic system in patients with AICI,and the damage of the glial lymphatic system in patients with AICI is in the same cerebral hemisphere.
