摘 要:目的:探究不同等级深度学习(DL)重建算法在低剂量CT结肠造影(CTC)中的临床应用价值。方法:前瞻性收集在我院接受CTC检查的患者38例,所有患者均行俯卧位和仰卧位两次扫描,根据扫描体位分为A、B两组。A组(俯卧位,常规组)行120 kV、剂量调制等级2扫描,重建Karl 5级图像;B组(仰卧位,实验组)行120 kV、剂量调制等级1扫描,重建DL 1~4级图像,分别记为B1~B4组。比较A、B两组平均辐射剂量及主客观图像质量。记录各组图像中肿瘤的位置,以手术结果为标准,比较各组图像肿瘤定位与手术结果的一致性。结果:B组平均mA数、容积CT剂量指数(CTDIvol)、有效辐射剂量(ED)较A组均显著降低(P<0.001),其中ED较A组减低约58%。A、B两组肝脏、腰大肌和脂肪的CT值差异无统计学意义(P>0.05);B1组的标准差(SD)值高于A组(P<0.001),信噪比(SNR)、对比度噪声比(CNR)则低于A组(P<0.001);B2组上述指标与A组差异无统计学意义(P>0.05);B3和B4组的SD值均低于A组(P<0.001),SNR、CNR均高于A组(P<0.001)。2名医师的主观评分一致性较好(Kappa值=0.721~0.842,P<0.001),B2组图像整体质量得分最高(P<0.001),与A组得分差异无统计学意义(P>0.05)。A、B两组的肿瘤影像定位与手术结果的一致性均较高(Kappa值=0.790,P<0.001)。结论:DL重建算法可降低图像噪声,减少有效辐射剂量,且采用DL 2级重建算法时,图像质量与常规组相当。Purpose:To assess the clinical utility of deep learning(DL)reconstruction algorithms in low-dose CT colonography(CTC).Methods:A total of 38 patients who underwent CTC scans in our hospital were prospectively collected.All patients were scanned twice in prone position and supine position,and were divided into group A and group B according to the scanning position.Group A(prone position,conventional group)underwent 120 kV,dose modulation level 2 scanning,and Karl 5 image reconstruction.Group B(supine position,experimental group)underwent 120 kV,dose modulation level 1 scanning,and DL 1-4 image reconstruction,which were recorded as group B1-B4.The average radiation dose and subjective and objective image quality were compared between group A and B.The location of the tumor in each group was recorded,and the consistency of the tumor location in each group was compared with the surgical results.Results:The average mA,volume CT dose index(CTDIvol),and effective dose(ED)in group B were significantly lower than those in group A(P<0.001).The ED in group B was reduced by about 58% compared with that in group A.There were no statistically significant differences in the CT values of liver,psoas muscle,and fat between group A and group B(P>0.05).The standard deviation(SD)value of group B1 was higher than that of group A(P<0.001).The signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)of group B1 were lower than those of group A(P<0.001).The above-mentioned values had no statistically significant difference between group B2 and group A(P>0.05).The SD values of group B3 or group B4 were lower than those of group A(P<0.001),while the SNR and CNR of group B3 or group B4 were higher than those of group A(P<0.001).The subjective scores of the two radiologists had good consistency(Kappa value 0.721-0.842,P<0.001).The overall image quality of group B2 scored the highest(P<0.001),and had no statistically significant difference with group A(P>0.05).The consistencies between tumor location and surgical results in group A and group
