摘　　要：目的探讨延迟皮瓣法耳再造术治疗耳甲腔型小耳畸形的临床效果。方法回顾性分析自2011年1月至2021年12月,湖南省人民医院整形激光美容科收治的52例耳甲腔型小耳畸形患者,采用延迟皮瓣法耳再造术,分二至三期进行,一期行耳后延迟皮瓣术,同时处理残耳;二期行肋软骨支架植入延迟耳皮瓣+耳后筋膜瓣+自体中厚皮移植覆盖耳再造术;三期修整再造耳。结果52例患者完成三期手术46例,仅行二期手术6例,所有患者切口均一期愈合,无感染、皮瓣(皮片)坏死等并发症发生。所有患者再造术后获随访6个月,32例满意,20例基本满意。结论采用延迟皮瓣法耳再造治疗耳甲腔型小耳畸形能获得较好术后效果,是耳甲腔型小耳畸形治疗方法的比较好的补充。Objective To explore the efficacy of the auricular reconstruction using delayed postauricular skin flap applied to concha-type microtia.Methods From January 2011 to December 2021,a retrospective study of patients with concha-type microtia,whom we accepted and treated with the auricular reconstruction using delayed postauricular skin flap.The reconstruction was performed in three stages.The first stage is the procedure of delayed postauricular skin flap preparing and residual auricle dealing,the second stage is the autogenous rib cartilage framework placed and covered with the delayed postauricular skin flap and a postauricular fascial flap which is covered by autologous medium-thickness skin graft,and the third stage involved modification of the reconstructed ear.Results There are a total of 52 cases,of which 46 have completed the third stage surgery and 6 have completed the second stage surgery.All the surgical incisions were primary healing,without infection and necrosis.Follow up observation for six months after surgery,there were 32 of them satisfied,20 basic satisfied,and no one dissatisfied.Conclusion The auricular reconstruction using delayed postauricular skin flap is a good supplement to the treatment for concha-type microtia.
