摘 要:目的:利用Meta分析系统评价1064 nm Nd∶YAG激光治疗婴幼儿血管瘤的疗效及安全性。方法:计算机检索PubMed、Med of science、中国知网(CNKI)、维普(VIP)期刊全文及万方电子期刊全文数据库截止至2022年9月10日所有关于1064 nm Nd∶YAG激光治疗婴儿血管瘤的随机对照研究。对最终纳入的所有研究进行质量评价,使用RevMan5.4.1软件进行Meta分析。结果:共纳入8个随机对照研究,835例血管瘤患儿。结果显示,1064 nm Nd∶YAG激光治疗血管瘤的显效率与单独口服普萘洛尔相比,差异无统计学意义(P>0.05),但其安全性较单独口服普萘洛尔低。单独使用1064 nm Nd∶YAG激光治疗血管瘤的显效率和安全性均低于联合口服普萘洛尔治疗;1064 nm Nd∶YAG激光治疗血管瘤的显效率高于单独局部使用噻吗洛尔,但安全性较单独局部使用噻吗洛尔低;单独使用1064 nm Nd∶YAG激光治疗血管瘤的显效率和安全性均低于激光联合噻吗洛尔(均P<0.05)。单独使用1064 nm Nd∶YAG激光治疗血管瘤与激光联合聚桂醇治疗显效率差异无统计学意义(P>0.05)。结论:1064 nm Nd∶YAG激光治疗婴幼儿血管瘤与传统药物治疗相比有着良好的疗效,但其安全性相对较低,与药物联合治疗可一定程度上提高治疗的显效率及安全性。Objective The meta-analysis system was used to evaluate the efficacy and safety of Nd:YAG 1064 nm laser in the treatment of infantile hemangioma.Methods Cochrane Library,PubMed,Med of science,CNKI,VIP and Wanfang database were searched.All relevant randomized controlled trials of Nd:YAG 1064 nm laser treatment of infant hemangioma,up to September 10,2022,were retrieved.Quality evaluation of all the studies were included.The meta analysis was performed using RevMan5.4.1 software.Results A total of 8 randomized controlled studies were enrolled,with a total of 835 cases of infantile hemangioma.The results showed that there was no statistically significant difference in the efficacy between the 1064 nm Nd:YAG laser therapy and propranolol alone in the treatment of hemangioma(P>0.05).The 1064 nm Nd:YAG laser therapy was less safer than propranolol alone in the treatment of hemangioma.The efficacy and safety of 1064 nm Nd:YAG laser alone in the treatment of hemangiomas were lower than that of combined oral propranolol treatment;the efficacy of 1064 nm Nd:YAG laser in the treatment of hemangiomas was higher than that of topical timolol alone,but the safety was lower than that of topical timolol alone;the efficacy and safety of 1064 nm Nd:YAG laser alone in the treatment of hemangiomas was The efficacy and safety of 1064 nm Nd:YAG laser alone in the treatment of hemangiomas are lower than that of laser combined with timolol(all P<0.05).There was no statistically significant difference in the efficiency between 1064 nm Nd:YAG laser therapy and 1064 nm Nd:YAG laser therapy combined with polycinnamon(P>0.05).Conclusion This meta-analysis provides strong evidences that Nd:YAG 1064 nm laser therapy of hemangioma was more effective and less safer than traditional drugs.Nd:YAG 1064 nm laser in combination with drugs can improve the efficiency and safety to some extent in the treatment of hemangioma.
